摘要
目的探究盐酸多奈哌齐联合重复经颅磁刺激(rTMS)对阿尔兹海默症(AD)患者认知功能及事件相关电位(ERP)P300的影响.方法选择本院2019年10月—2021年8月收治的AD患者92例,按照随机数字表法分为对照组和观察组,每组46例.对照组口服盐酸多奈哌齐治疗,观察组在此基础上加用rTMS治疗,均持续治疗3个月.比较两组的认知功能、ERP-P300及不良反应.结果治疗前,两组的认知功能、ERP-P300比较,组间差异无统计学意义(P>0.05);治疗后,观察组的简易精神状态评定表中定向力、语言能力、即刻记忆力、计算和注意力及回忆力评分分别为(7.53±1.36)分、(6.94±1.20)分、(1.92±0.38)分、(4.11±0.41)分、(2.11±0.34)分,均高于对照组,N2潜伏期、P3潜伏期分别为(221.46±23.63)ms、(316.26±32.53)ms,均短于对照组,P3波幅为(11.37±2.26)μv,高于对照组,组间差异有统计学意义(P<0.05).两组均未出现严重不良反应.结论AD患者采用盐酸多奈哌齐联合rTMS治疗的效果确切,能够改善认知功能,调节ERP-P300,且安全性高.
Objective To investigate the effects of donepezil hydrochloride combined with repetitive transcranial magnetic stimulation(rTMS)on cognitive function and event-related potentials P300 in patients with Alzheimer’s disease(AD).Methods A total of 92 AD patients admitted to our hospital from October 2019 to August 2021 were divided into a control group and an observation group according to random number table method,with 46 cases in each group.The control group was treated with donepezil hydrochloride orally,and the observation group was additionally treated with r TMS.Both groups were treated for 3 months.The cognitive function,ERP-P300 and adverse reactions of the two groups were compared.Results Before treatment,there were no significant differences in cognitive function and ERP-P300 between the two groups(P>0.05);after treatment,the scores of orientation,language ability,immediate memory,calculation,attention and recall in the mini-mental state examination of the observation group were(7.53±1.36)points,(6.94±1.20)points,(1.92±0.38)points,(4.11±0.41)points,(2.11±0.34)points,respectively,which were higher than those in the control group,the incubation periods of N2and P3 were(221.46±23.63)ms and(316.26±32.53)ms,respectively,which were shorter than those of the control group,the amplitude of P3 was(11.37±2.26)μV,which was higher than that of the control group,and the differences between the groups were statistically significant(P<0.05).There were no serious adverse reactions in both groups.Conclusion Donepezil hydrochloride combined with r TMS has a definite effect on AD patients,which can improve cognitive function and regulate ERP-P300 with high safety.
