摘要
目的探讨精密型摄食训练监管护理在帕金森病(PD)吞咽障碍患者中的应用效果.方法选取2020年8月—2022年5月我院收治的94例PD吞咽障碍患者,按随机数字表法分为两组,各47例.对照组采用常规护理,观察组采用精密型摄食训练监管护理,均持续护理8周.对比两组吞咽功能、摄食功能、并发症发生率及生存质量.结果护理前,两组吞咽功能、摄食功能比较,组间差异无统计学意义(P>0.05);护理后,观察组标准吞咽功能评价量表评分为(20.72±2.44)分,低于对照组的(25.60±3.35)分,功能性经口摄食量表评分为(6.09±0.28)分,高于对照组的(5.48±0.39)分,组间差异有统计学意义(P<0.05).观察组并发症发生率为4.26%,低于对照组的19.15%,差异有统计学意义(P<0.05).护理前,两组生活质量比较,差异无统计学意义(P>0.05);护理后,观察组生活质量综合评定问卷中的心理功能评分(75.01±5.43)分、躯体功能评分(74.08±4.30)分、社会功能评分(72.20±5.16)分、物质生活状态评分(78.66±4.49)分,均高于对照组的(67.33±5.19)分、(66.88±5.97)分、(65.79±6.12)分、(72.30±4.26)分,组间差异有统计学意义(P<0.05).结论精密型摄食训练监管护理用于PD吞咽障碍患者中能改善其吞咽与摄食功能,减少并发症的发生,并提高生存质量.
Objective To investigate the effect of supervised care of precision-type feeding training in patients with swallowing disorders in Parkinson's disease(PD).Methods 94 patients with PD swallowing disorder admitted to the hospital from August 2020 to May 2022 were selected and divided into two groups of 47 cases each according to the random number table method.The control group used conventional care and the observation group used precision-type feeding training supervised care,and both continued care for 8 weeks.Swallowing function,ingestion function,complication rate and survival quality were compared between the two groups.Results Before care,there were no statistically significant differences between the two groups in the comparison of swallowing function and feeding function(P>0.05);after care,the standard swallowing function evaluation scale score of the observation group was(20.72±2.44)points,which was lower than that of the control group(25.60±3.35)points,and the functional transoral feeding scale score was(6.09±0.28)points,which was higher than that of the control group(5.48±0.39)points,and the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 4.26%,which was lower than 19.15%of the control group,and the difference was statistically significant(P<0.05).Before care,there was no statistically significant difference in quality of life between the two groups(P>0.05);after care,the psychological function score(75.01±5.43)points,physical function score(74.08±4.30)points,social function score(72.20±5.16)points,and material life status score(78.66±4.49)points of the generic quality of life inventory-74 in the observation group were higher than those of the control group(67.33±5.19)points,(66.88±5.97)points,(65.79±6.12)points,and(72.30±4.26)points in the control group,with statistically significant differences between the groups(P<0.05).Conclusion Precision-type feeding training supervised care for patients with PD swallowing disorders improves swallowing and feeding function,reduces complications,and improves quality of survival.
