天麻钩藤饮加减联合硝苯地平对高血压患者中医症候积分及血压变异性参数的影响被引量：4

Effect of Tianma Gouteng Yin Combined with Nifedipine on TCM Syndrome Score and Blood Pressure Variability Parameters in Patients with Hypertension

摘要目的探究天麻钩藤饮加减联合硝苯地平在高血压患者中的具体效用.方法选取2020年6月—2022年6月该院诊治的68例高血压患者,按随机数字表法分为两组,各34例.对照组予以硝苯地平,观察组在对照组基础上加用天麻钩藤饮加减,连续治疗4周.对比两组临床疗效、中医症候积分、血压变异性参数、不良反应.结果观察组治疗总有效率94.12%高于对照组的76.47%,差异有统计学意义(P<0.05).治疗前,两组头晕头痛、烦躁易怒、面红耳赤、口干口苦积分与24 h平均收缩压(24 h SBP)、24 h平均舒张压(24 h DBP)、24 h收缩压标准差(24 h SSD)、24 h舒张压标准差(24 h DSD)相比,组间差异无统计学意义(P>0.05);治疗4周后,观察组头晕头痛积分为(0.97±0.21)分、烦躁易怒积分为(0.59±0.11)分、面红耳赤积分为(0.69±0.12)分、口干口苦积分为(0.71±0.15)分、24 h SBP为(129.48±9.31)mmHg、24 h DBP为(75.60±4.89)mmHg、24 h SSD为(7.21±1.04)mmHg、24 h DSD为(6.31±1.05)mmHg,均低于对照组的(1.53±0.36)分、(1.22±0.30)分、(1.38±0.18)分、(1.42±0.20)分、(135.31±10.75)mmHg、(83.21±5.30)mmHg、(12.41±1.53)mmHg、(7.74±1.24)mmHg,组间差异有统计学意义(P<0.05).两组治疗期间均无不良反应发生.结论天麻钩藤饮加减联合硝苯地平在高血压患者治疗中作用明显,可有效减轻其各项症状,降低血压变异性,且无不良反应,具有较高的应用价值. Objective To explore the specific efficacy of Tianma Gouteng Yin combined with nifedipine in patients with hypertension.Methods 68 patients with hypertension diagnosed and treated in the hospital from June 2020 to June 2022 were randomly divided into two groups,with 34 patients in each group.The control group was treated with nifedipine,and the observation group was treated with Tianma Gouteng Yin on the basis of the control group,with both interventions for 4 weeks.The clinical efficacy,TCM syndrome score,blood pressure variability parameters and adverse reactions of the two groups were compared.Results The total effective rate of treatment of observation group was 94.12%,higher than 76.47%of control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in the scores of dizziness and headache,fretful and irritability,red face and red ear,dry mouth and bitter mouth,24 h mean systolic blood pressure(24 h SBP),24 h mean diastolic blood pressure(24 h DBP),24 h systolic blood pressure standard deviation(24 h SSD),24 h diastolic blood pressure standard deviation(24 h DSD)between the two groups(P>0.05);after treatment,the score of dizziness and headache in the observation group was(0.97±0.21)points,the score of fretful and irritability was(0.59±0.11)points,the score of red face and red ear was(0.69±0.12)points,the score of dry mouth and bitter mouth was(0.71±0.15)points,the 24 h SBP was(129.48±9.31)mmHg,the 24 h DBP was(75.60±4.89)mmHg,the 24 h SSD was(7.21±1.04)mmHg,and the 24 h DSD was(6.31±1.05)mmHg,which were lower than(1.53±0.36)points,(1.22±0.30)points,(1.38±0.18)points,(1.42±0.20)points,(135.31±10.75)mmHg,(83.21±5.30)mmHg,(12.41±1.53)mmHg,(7.74±1.24)mmHg of control group,the differences between the groups were statistically significant(P<0.05).No adverse reaction occurred in both groups during treatment.Conclusion Tianma Gouteng Yin combined with nifedipine has an obvious effect in the treatment of hypertension patients,which can effectively reduce their symptoms,reduce blood pressure variability,and has no adverse reactions.It has high application value.
