摘要
目的探讨促性腺激素释放激素激动剂(GnRH-a)联合腹腔镜手术治疗子宫内膜异位症(EMs)伴不孕症的效果.方法选择2017年9月—2020年1月本院收治的EMs伴不孕症患者78例,按随机数字表法分为两组,每组39例.两组均行腹腔镜手术,在此基础上,对照组采用孕激素治疗,观察组采用GnRH-a治疗,均治疗6个月,并持续随访2年.对比两组的卵巢功能、主观症状、不良反应发生情况及妊娠情况.结果治疗后,观察组的血清卵泡刺激素、雌二醇及促黄体生成素水平分别为(5.31±0.49)pg/mL、(52.13±5.85)mIU/mL、(4.19±0.40)mIU/mL,均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的痛经、性交痛及慢性盆腔痛评分分别为(0.98±0.19)分、(0.94±0.25)分、(0.88±0.18)分,均低于对照组,组间差异有统计学意义(P<0.05).观察组的不良反应发生率为5.13%,低于对照组的23.08%,妊娠率为89.74%,高于对照组的61.54%,组间差异有统计学意义(P<0.05).结论GnRH-a联合腹腔镜手术在EMs伴不孕症患者中具有较好的应用效果,可改善其卵巢功能,减轻主观症状,改善妊娠情况,且不良反应发生率较低,应用安全性较好.
Objective To investigate the efficacy of gonadotropin-releasing hormone agonist(GnRH-a)combined with laparoscopic surgery in patients with endometriosis(EMs)and infertility.Methods A total of 78 EMs patients with infertility admitted to our hospital from September 2017 to January 2020 were selected and were divided into two groups according to random number table method,with 39 cases in each group.Both groups underwent laparoscopic surgery.On this basis,the control group was treated with progesterone,and the observation group was treated with GnRH-a.Both groups were treated for 6 months and followed up for 2 years.The ovarian function,subjective symptoms,occurrence of adverse reactions and pregnancy were compared between the two groups.Results After treatment,the serum levels of follicle stimulating hormone,estradiol and luteinization hormone in the observation group were(5.31±0.49)pg/mL,(52.13±5.85)mIU/mL and(4.19±0.40)mIU/mL,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the scores of dysmenorrhea,dysparegia and chronic pelvic pain in the observation group were(0.98±0.19)points,(0.94±0.25)points and(0.88±0.18)points,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.13%,lower than 23.08%in the control group,and the pregnancy rate was 89.74%,higher than 61.54%in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion GnRH-a combined with laparoscopic surgery has a good application effect in EMs patients with infertility,which can improve ovarian function,alleviate subjective symptoms and improve pregnancy conditions,and the incidence of adverse reactions is low,and the application safety is good.
