摘要
目的探讨亮丙瑞林在卵巢巧克力囊肿腹腔镜术后患者中的应用效果.方法选取2020年5月—2022年5月我院收治的82例卵巢巧克力囊肿患者为研究对象,按随机数字表法分为两组,各41例.对照组采用腹腔镜手术治疗,观察组在对照组基础上术后加用亮丙瑞林治疗.比较两组患者血清性激素水平、子宫体积、子宫内膜厚度、月经情况、卵巢血供情况、生活质量.结果观察组术后3个月卵泡刺激素、促黄体生成素水平分别为(28.36±3.19)IU/L、(16.53±2.04)IU/L,均高于对照组,雌二醇、孕酮水平分别为(42.36±5.14)pmol/L、(1.03±0.11)pmol/L,均低于对照组,组间差异有统计学意义(P<0.05);观察组术后3个月子宫体积为(96.52±7.15)cm3,小于对照组,子宫内膜厚度为(5.71±1.05)mm,薄于对照组,组间差异有统计学意义(P<0.05);观察组术后3个月经期时长为(5.12±0.82)d,短于对照组,疼痛视觉模拟评分为(1.85±0.23)分,低于对照组,组间差异有统计学意义(P<0.05);观察组术后3个月卵巢基质动脉舒张末期最低血流速度、收缩期峰值流速分别为(4.38±0.52)cm/s、(15.89±1.58)cm/s,均快于对照组,组间差异有统计学意义(P<0.05);观察组术后3个月世界卫生组织生活质量测定量表简表中生理、心理、社会及环境领域评分分别为(90.61±4.27)分、(93.74±3.82)分、(89.89±5.18)分、(93.58±2.14)分,均高于对照组,组间差异有统计学意义(P<0.05).结论亮丙瑞林能够改善卵巢巧克力囊肿腹腔镜术后患者的血清性激素水平,加快子宫体积及子宫内膜厚度恢复,改善卵巢血供,纠正月经紊乱,提高生活质量.
Objective To investigate the application effect of leprerelin in patients with ovarian chocolate cyst after laparoscopic surgery.Methods 82 patients with ovarian chocolate cyst admitted to the hospital from May 2020 to May 2022 were selected as the research objects and divided into two groups according to random number table method,with 41 cases in each group.The control group was treated by laparoscopy,and the observation group was treated with leuprolide after surgery on the basis of the control group.Serum sex hormone level,uterine volume,endometrial thickness,menstruation,ovarian blood supply and quality of life were compared between the two groups.Results 3 months after operation,the levels of follicle stimulating hormone and luteinizing hormone in the observation group were(28.36±3.19)IU/L,(16.53±2.04)IU/L,respectively,which were higher than those in the control group,the levels of estradiol and progesterone were(42.36±5.14)pmol/L,(1.03±0.11)pmol/L,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);3 months after operation,the uterine volume of the observation group was(96.52±7.15)cm3,which was smaller than that of the control group,the endometrial thickness of the observation group was(5.71±1.05)mm,which was thinner than that of the control group,and the differences between the groups were statistically significant(P<0.05);3 months after operation,the duration of menstrual period in the observation group was(5.12±0.82)days,which was shorter than that in the control group,and the pain visual analogue scale score was(1.85±0.23)points,which was lower than that in the control group,and the differences between the groups were statistically significant(P<0.05);the end diastolic velocity and peak systolic velocity of ovarian stromal artery in the observation group were(4.38±0.52)cm/s and(15.89±1.58)cm/s at 3 months after operation,respectively,which were significantly faster than those in the control group,and the differences between the groups were statistically significant(P<0.05);the scores of physiological,psychological,social and environmental fields of World Health Organization's quality of life questionnaire-brief in the observation group were(90.61±4.27)points,(93.74±3.82)points,(89.89±5.18)points and(93.58±2.14)points,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Leuprorelin can improve the level of serum sex hormones,accelerate the recovery of uterine volume and endometrial thickness,improve ovarian blood supply,correct menstrual disorders,and improve the quality of life in patients with ovarian chocolate cyst after laparoscopic surgery.
