摘要
目的探讨可变优先级认知-运动双任务训练联合Bobath技术应用于脑卒中患者康复治疗中的效果.方法选取无锡市新吴区康复医院2020年1月—2022年9月收治的68例脑卒中患者为研究对象,通过摸球法随机分为两组.对照组(n=34)采用Bobath技术治疗,联合组(n=34)在对照组基础上采用可变优先级认知-运动双任务训练治疗.比较两组患者的平衡和肢体运动功能、神经和认知功能、等速肌力测试指标及临床疗效.结果治疗后,联合组的Berg平衡量表、Fugl-Meyer运动功能评定量表评分分别为(52.03±2.81)分、(75.82±9.51)分,均高于对照组的(49.11±2.60)分、(66.27±9.02)分,组间差异有统计学意义(P<0.05).治疗后,联合组的美国国立卫生研究院卒中量表评分为(21.62±3.88)分,低于对照组的(25.31±3.50)分,蒙特利尔认知量表评分为(26.30±3.19)分,高于对照组的(22.51±2.85)分,组间差异有统计学意义(P<0.05).治疗后,联合组的总功、峰力矩、平均功率分别为(221.57±31.58)J、(22.57±5.17)N·m、(16.07±4.05)W,均高于对照组的(167.42±26.17)J、(17.29±4.29)N·m、(11.24±3.59)W,组间差异有统计学意义(P<0.05).联合组的治疗总有效率为97.06%,高于对照组的73.53%,差异有统计学意义(P<0.05).结论脑卒中患者康复治疗中采用可变优先级认知-运动双任务训练联合Bobath技术的效果显著,能明显改善平衡和肢体运动功能、神经和认知功能,提高等速肌力测试指标,值得临床推广使用.
Objective To investigate the effect of variable priority cognitive-motor dual task training combined with Bobath technology in the rehabilitation of stroke patients.Methods 68 stroke patients admitted to the Wuxi Xinwu District Rehabilitation Hospital from January 2020 to September 2022 were selected as the research objects and were randomly divided into two groups by ball-touching method.The control group(n=34)was treated with Bobath technology,and the combination group(n=34)was treated with variable priority cognitive-motor dual task training on the basis of control group.The balance and limb motor function,neurological and cognitive function,isokinetic muscle strength test indexes and clinical efficacy were compared between the two groups.Results After treatment,the scores of Berg balance scale and Fugl-Meyer assessment in the combination group were(52.03±2.81)points and(75.82±9.51)points,respectively,higher than(49.11±2.60)points and(66.27±9.02)points in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the National Institutes of Health stroke scale score of the combined group was(21.62±3.88)points,lower than(25.31±3.50)points of the control group,and the Montreal cognitive assessment score was(26.30±3.19)points,higher than(22.51±2.85)points of the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the total work,peak torque and average power of the combined group were(221.57±31.58)J,(22.57±5.17)N·m and(16.07±4.05)W,respectively,which were higher than(167.42±26.17)J,(17.29±4.29)N·m and(11.24±3.59)W of the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of the combined group was 97.06%,higher than 73.53%of the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of variable priority cognitive-motor dual task training combined with Bobath technology is significant in the rehabilitation of stroke patients,which can significantly improve balance and limb motor function,nerve and cognitive function,and improve the isokinetic muscle strength test indexes,worthy of clinical promotion and use.
