摘要
目的观察基于容积-黏度吞咽测试的摄食训练对养老机构老年吞咽困难患者康复效果的影响。方法选取2021年9月—2022年9月养老机构中的70例老年吞咽障碍患者为研究对象,随机分为两组,每组35例。对照组进行常规吞咽功能训练,观察组在此基础上进行基于容积-黏度吞咽测试的摄食训练。对比两组患者的吞咽障碍程度、营养状态及生活质量。结果干预3个月后,观察组的吞咽障碍造影评分量表中口腔期、咽期评分分别为(29.12±3.18)分、(31.45±3.26)分,均低于对照组的(31.70±4.36)分、(33.90±3.34)分,组间差异有统计学意义(P<0.05)。干预3个月后,观察组的血清白蛋白、血红蛋白水平分别为(118.54±13.21)g/L、(35.28±4.44)g/L,均高于对照组的(109.24±13.36)g/L、(31.93±4.49)g/L,组间差异有统计学意义(P<0.05)。干预3个月后,观察组的安德森吞咽困难量表中生理、功能、情感、总体状况评分均高于对照组,组间差异有统计学意义(P<0.05)。结论基于容积-黏度吞咽测试的摄食训练在养老机构老年吞咽困难患者中的应用效果确切,能减轻其吞咽障碍,改善营养状态,提升生活质量。
Objective To observe the effect of feeding training based on volumetric viscosity swallowing test on rehabilitation of elderly patients with dysphagia in care institutions.Methods 70 elderly patients with dysphagia in care institutions from September 2021 to September 2022 were selected as the research objects and were randomly divided into two groups,with 35 cases in each group.The control group underwent routine swallowing function training,and the observation group underwent feeding training based on volume-viscosity swallowing test on this basis.The degree of dysphagia,nutritional status and quality of life of the two groups were compared.Results After 3 months of intervention,the oral stage and pharyngeal stage scores of the videofluoroscopic dysphagia scale in the observation group were(29.12±3.18)points and(31.45±3.26)points,respectively,which were lower than(31.70±4.36)points and(33.90±3.34)points of the control group,and the differences between the groups were statistically significant(P<0.05).After 3 months of intervention,the serum albumin and hemoglobin levels in the observation group were(118.54±13.21)g/L and(35.28±4.44)g/L,respectively,which were higher than(109.24±13.36)g/L and(31.93±4.49)g/L in the control group,the differences between the groups were statistically significant(P<0.05).After 3 months of intervention,the physiological,functional,emotional and general status scores of the M.D.Anderson Dysphagia Inventory in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The feeding training based on volumetric viscosity swallowing test is effective in elderly patients with dysphagia,which can alleviate dysphagia,improve nutritional status and improve quality of life.
