摘要
目的研究急性脑梗死患者采用机械介入取栓术与重组组织型纤溶酶原激活剂(rt-PA)溶栓治疗对其血清相关指标及神经功能的影响。方法回顾性分析2020年1月—2021年1月本院神经外科收治的110例急性脑梗死患者的病例资料,根据不同治疗方案分为rt-PA组与联合组,各55例。rt-PA组采用单一rt-PA溶栓治疗,联合组在rt-PA组基础上实施机械介入取栓术。比较两组治疗前、治疗后6、24、72 h神经功能以及治疗前、治疗后7 d血清相关指标。结果治疗后6、24、72 h,联合组mRS评分、NIHSS评分均低于rt-PA组,组间差异有统计学意义(P<0.05);治疗后7 d,联合组CRP、IL-6、NSE、Lp-PLA2水平均低于rt-PA组,PON-1水平高于rt-PA组,组间差异有统计学意义(P<0.05)。结论在rt-PA溶栓治疗的基础上联合机械介入取栓术取栓,对改善急性脑梗死患者的神经功能、减轻体内炎症反应和保护血管功能具有更显著的效果。
Objective To study the effects of mechanical interventional embolectomy and recombinant tissue plasminogen activator(rt-PA)thrombolysis on serum related indexes and neurological function in patients with acute cerebral infarction.Methods The clinical data of 110 patients with acute cerebral infarction from January 2020 to January 2021 in our neurosurgery department were retrospectively analyzed.According to different treatment schemes,they were divided into a rt-PA group and a combined group,with 55 cases in each group.The control group was treated with single rt-PA thrombolysis,while the combined group was treated with mechanical interventional embolectomy on the basis of control group.The neurological function before treatment,6,24 and 72 h after treatment and serum related indexes before treatment and 7 d after treatment were compared between the two groups.Results 6,24 and 72 h after treatment,the scores of mRS and NIHSS in the combined group were lower than those in the rt-PA group,and the differences between the groups were statistically significant(P<0.05).7 d after treatment,the levels of CRP,IL-6,NSE and Lp-PLA2 in combined group were lower than those in the rt-PA group,and the level of PON-1 was higher than that in the rt-PA group,with statistical significance between the two groups(P<0.05).Conclusion On the basis of rt-PA thrombolytic therapy combined with mechanical interventional embolectomy,it has a more significant effect on improving neurological function,reducing inflammatory reaction and protecting vascular function in patients with acute cerebral infarction.
