摘要
目的探讨不同制剂型肠内营养支持治疗对直肠癌根治术后患者营养指标、炎症因子、胃肠道功能的影响。方法选取2018年2月—2023年2月山东省枣庄市峄城区人民医院综合内科收治的70例行直肠癌根治术的患者为研究对象,按随机数字表法分为两组。对照组(n=35)采用整蛋白型肠内营养,观察组(n=35)采用氨基酸型、短肽型及整蛋白型肠内营养。对比两组营养指标、炎症因子、胃肠道功能。结果治疗后,观察组白蛋白(28.02±4.29)g/L、前白蛋白(175.58±35.22)mg/L、总蛋白(66.72±7.58)g/L均高于对照组的(24.86±4.11)g/L、(147.96±32.11)mg/L、(60.31±7.34)g/L;观察组的白细胞介素6(251.22±31.57)ng/L、白细胞介素8(41.93±9.12)pg/mL、肿瘤坏死因子-α(182.64±31.07)ng/L、C反应蛋白(9.20±2.69)mg/L、生长抑素(46.77±3.68)ng/L均低于对照组的(339.02±40.58)ng/L、(53.15±10.56)pg/mL、(241.88±36.02)ng/L、(11.96±2.97)mg/L、(53.12±4.12)ng/L;观察组的胃动素(65.49±10.21)ng/L、胃泌素(119.04±24.66)ng/L、血管活性肠肽(35.02±6.89)ng/L均高于对照组的(53.58±11.20)ng/L、(84.16±17.03)ng/L、(27.16±8.11)ng/L,组间差异有统计学意义(P<0.05)。结论应用氨基酸型、短肽型及整蛋白型肠内营养治疗直肠癌根治术后患者能够明显加快患者康复速度,改善患者的胃肠道功能。
Objective To explore the effects of different types of enteral nutrition support on nutritional indicators,inflammatory factors,and gastrointestinal function of patients after radical resection of rectal cancer.Methods 70 patients with rectal cancer who had undergone radical resection in the General Internal Medicine Department of Yicheng District People's Hospital,Zaozhuang City,Shandong Province from February 2018 to February 2023 were selected as the research objects,and were divided into two groups according to the random number table.The control group(n=35)received whole protein enteral nutrition,while the observation group(n=35)received amino acid,short peptide,and whole protein enteral nutrition.Compare the inflammatory factors,and gastrointestinal function of the two groups of patients.Results After treatment,the levels of albumin(28.02±4.29)g/L,prealbumin(175.58±35.22)mg/L,and total protein(66.72±7.58)g/L in the observation group were higher than(24.86±4.11)g/L,(147.96±32.11)mg/L,(60.31±7.34)g/L in the control group;the levels of interleukin-6(251.22±31.57)ng/L,interleukin-8(41.93±9.12)pg/mL,tumor necrosis factor-α(182.64±31.07)ng/L,C-reactive protein(9.20±2.69)mg/L,and somatostatin(46.77±3.68)ng/L in the observation group were lower than(339.02±40.58)ng/L,(53.15±10.56)pg/mL,(241.88±36.02)ng/L,(11.96±2.97)mg/L,and(53.12±4.12)ng/L in the control group;the levels of motilin(65.49±10.21)ng/L,gastrin(119.04±24.66)ng/L,and vasoactive intestinal peptide(35.02±6.89)ng/L in the observation group were higher than(53.58±11.20)ng/L,(84.16±17.03)ng/L,(27.16±8.11)ng/L in the control group,the difference was statistically significant(P<0.05).Conclusion The use of amino acid,short peptide,and whole protein enteral nutrition therapy can significantly accelerate the recovery rate of patients and improve their gastrointestinal function after radical resection of rectal cancer.
