摘要
目的探究屈螺酮炔雌醇片(Ⅱ)联合二甲双胍对多囊卵巢综合征(PCOS)患者卵巢功能及胰岛功能的影响.方法选取2021年4月—2022年5月平原县第一人民医院收治的76例PCOS患者为研究对象,按随机数字表法将其分为对照组(n=38)及观察组(n=38).对照组采用屈螺酮炔雌醇片(Ⅱ)治疗,观察组在此基础上采用二甲双胍治疗,均持续3个月.对比两组患者的血脂水平、胰岛及卵巢功能、不良反应发生情况.结果治疗后,观察组的雌二醇、睾酮、黄体生成素、黄体生成素/卵泡刺激素水平分别为(107.01±18.29)pmol/L、(1.66±0.49)mmol/L、(8.62±1.81)U/L、(1.55±0.65),均低于对照组,组间差异有统计学意义(P<0.05);观察组的空腹胰岛素、C肽水平分别为(9.02±1.96)U/L、(0.41±0.08)nmol/L,均低于对照组,组间差异有统计学意义(P<0.05);观察组的三酰甘油、总胆固醇、低密度脂蛋白胆固醇水平分别为(1.28±0.28)mmol/L、(3.94±0.69)mmol/L、(2.41±0.33)mmol/L,均低于对照组,高密度脂蛋白胆固醇水平为(1.38±0.27)mmol/L,高于对照组,组间差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论PCOS患者采用屈螺酮炔雌醇片(Ⅱ)联合二甲双胍治疗可改善卵巢、胰岛功能,调节血脂水平,且不良反应较少,值得临床推广使用.
Objective To investigate the effects of drospirenone ethinylestradiol tablets(Ⅱ)combined with metformin on ovarian function and pancreatic islet function in patients with polycystic ovary syndrome(PCOS).Methods 76 patients with PCOS admitted to the Pingyuan County First People's Hospital from April 2021 to May 2022 were selected as the study objects and were divided into control group(n=38)and observation group(n=38)according to random number table method.The control group was treated with drospirenone ethinylestradiol tablets(Ⅱ),and the observation group was treated with metformin on this basis.Both groups were treated for 3 months.The lipid index levels,islet and ovarian function and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the levels of estradiol,testosterone,luteinizing hormone,luteinizing hormone/follicle stimulating hormone in the observation group were(107.01±18.29)pmol/L,(1.66±0.49)mmol/L,(8.62±1.81)U/L and(1.55±0.65),respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the fasting insulin and C-peptide levels in the observation group were(9.02±1.96)U/L and(0.41±0.08)nmol/L,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the levels of triglyceride,total cholesterol and low density lipoprotein-cholesterol in the observation group were(1.28±0.28)mmol/L,(3.94±0.69)mmol/L and(2.41±0.33)mmol/L,respectively,which were lower than those in the control group,while the level of high density lipoprotein-cholesterol was(1.38±0.27)mmol/L,which was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of PCOS patients with drospirenone ethinylestradiol tablets(Ⅱ)combined with metformin can improve ovarian and islet functions,regulate lipid levels,and have less adverse reactions,which is worthy of clinical promotion.
