摘要
目的探究吗替麦考酚酯联合泼尼松对狼疮性肾炎患者肾功能及免疫功能的影响.方法选取2020年1月—2022年9月临沂市中心医院收治的98例狼疮性肾炎患者为研究对象,按随机数字表法将其分为对照组及观察组,每组49例.对照组采用泼尼松及羟氯喹治疗,观察组在此基础上采用吗替麦考酚酯治疗,均持续6个月.比较两组患者的临床疗效、肾功能指标、免疫功能指标、抗双链DNA(ds-DN)抗体阳性率及不良反应发生情况.结果观察组的治疗总有效率为94.87%,高于对照组的73.46%,差异有统计学意义(P<0.05).治疗后,观察组的血清白蛋白及T淋巴细胞亚群CD4+水平均高于对照组,尿素氮、血肌酐、免疫球蛋白A及免疫球蛋白M水平均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的抗ds-DN抗体阳性率为14.29%,低于对照组的38.78%,差异有统计学意义(P<0.05).两组的不良反应总发生率比较,差异无统计学意义(P>0.05).结论采用吗替麦考酚酯联合泼尼松治疗狼疮性肾炎的效果显著,可改善患者肾功能及免疫功能,抑制其体内抗ds-DNA抗体表达,且安全性良好.
Objective To explore the effects of mycophenolate mofetil combined with prednisone on renal and immune function in patients with lupus nephritis.Methods 98 patients with lupus nephritis admitted to Linyi Central Hospital from January 2020 to September 2022 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 49 cases in each group.The control group was treated with prednisone and hydroxychlor-oquine,and the observation group was treated with mycophenolate mofetil on this basis.Both groups were treated for 6 months.The clinical efficacy,renal function indexes,immune function indexes,anti-doublestranded DNA(ds-DN)antibody positive rate and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.87%,which was higher than 73.46%of the control group,the difference was statistically significant(P<0.05).After treatment,the level of serum albumin and T lymphocyte subsets CD4+in observation group were higher than that in the control group,and the levels of urea nitrogen,serum creatinine,immunoglobulin A and immunog-lobulin M were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the positive rate of anti-ds-DN antibody in the observation group was 14.29%,lower than 38.78%in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Mycophenolate mofetil combined with prednisone is effective in the treatment of lupus nephritis,which can improve renal and immune function,inhibit the expression of anti-ds-DNA antibody,and has good safety.
