摘要
目的分析头皮针配合Schuell刺激法言语训练在中风后失语症患者中的应用效果。方法选取2021年9月-2023年7月淮北市中医医院收治的80例中风后失语症患者为研究对象,按随机数字表法将其分为对照组与观察组,各40例。对照组行Schuell刺激法言语训练,观察组在对照组基础上加以头皮针治疗,持续治疗4周。对比两组患者的临床疗效、血液流变学指标及语言功能恢复情况。结果观察组治疗总有效率为95.00%,高于对照组的77.50%,差异有统计学意义(P<0.05)。治疗前,两组全血低切粘度、全血高切粘度、红细胞压积(HCT)与西方失语症评定量表(WAB)内各维度评分及总分相比,组间差异无统计学意义(P>0.05);治疗后,观察组全血低切粘度为(8.30±1.42)mPa·s,全血高切粘度为(4.24±0.59)mPa·s,HCT为(40.49±4.25)%,均低于对照组的(11.49±2.01)mPa·s、(5.31±0.75)mPa·s、(47.15±5.71)%,WAB内各维度评分及总分分别为(14.88±2.01)分、(68.45±5.44)分、(78.95±6.77)分、(162.28±14.22)分,均高于对照组的(11.59±2.16)分、(60.71±5.26)分、(69.83±6.21)分、(142.13±13.63)分,组间差异有统计学意义(P<0.05)。结论头皮针配合Schuell刺激法言语训练可调节中风后失语症患者血液流变学,促进语言功能恢复。
Objective To analyze the effect of scalp acupuncture combined with Schuell stimulation speech training in patients with aphasia after stroke.Methods 80 patients with post-stroke aphasia treated in Huaibei Hospital of Traditional Chinese Medicine from September 2021 to July 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 40 cases in each group.The control group received Schuell stimulation speech training,and the observation group received scalp acupuncture on the basis of the control group and continued treatment for 4 weeks.The clinical efficacy,hemorheology index and speech function recovery were compared between the two groups.Results The total effective rate of the observation group was 95.00%,which was higher than 77.50%of the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference between the two groups in whole blood low shear viscosity,whole blood high shear viscosity,hematocrit(HCT)and Western Aphasia Rating Scale(WAB)scores and total scores(P>0.05);after treatment,the whole blood low shear viscosity was(8.30±1.42)mPa·s,the whole blood high shear viscosity was(4.24±0.59)mPa·s,and the HCT was(40.49±4.25)%,lower than(11.49±2.01)mPa·s,(5.31±0.75)mPa·s,(47.15±5.71)%of the control group,WAB scores and total scores were(14.88±2.01)points,(68.45±5.44)points,(78.95±6.77)points,and(162.28±14.22)points,respectively,it was higher than(11.59±2.16)points,(60.71±5.26)points,(69.83±6.21)points,(142.13±13.63)points of the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Scalp acupuncture combined with Schuell stimulation speech training can regulate blood rheology and promote language function recovery in patients with aphasia after stroke.
