摘要
目的探讨通关利窍针刺法联合吞咽障碍电刺激在缺血性脑卒中后吞咽障碍患者中的应用效果.方法选取该院2021年6月—2023年5月收治的102例缺血性脑卒中后吞咽障碍患者为研究对象,根据随机数字表法将纳入对象分为对照组和研究组,每组51例.对照组行常规吞咽功能训练,研究组在对照组基础上行通关利窍针刺法联合吞咽障碍电刺激.比较两组患者的临床疗效及治疗前、治疗30d后的表面肌电图、吞咽生活质量量表(SWAL-QOL)评分.结果研究组吞咽功能治疗总有效率96.08%高于对照组的78.43%,差异有统计学意义(P﹤0.05);治疗前,两组咽喉部位表面肌电活力水平对比,差异无统计学意义(P﹥0.05);治疗30d后,研究组最大波幅值高于对照组,吞咽时长短于对照组,组间差异有统计学意义(P﹤0.05);治疗前,两组SWAL-QOL评分比较,差异无统计学意义(P﹥0.05);治疗30d后,研究组SWAL-QOL评分高于对照组,差异有统计学意义(P﹤0.05).结论缺血性脑卒中后吞咽障碍患者在常规康复治疗基础上行通关利窍针刺法联合吞咽障碍电刺激,可显著提高吞咽障碍的治疗效果,增强咽喉部表面肌肉功能,从而提高患者的生活质量.
Objective Exploring and analyzing the application effect of Tongguan Liqiao acupuncture combined with electrical stimulation for swallowing disorders in patients with swallowing disorders after ischemic stroke.Methods 102 patients with swallowing disorders after ischemic stroke admitted to the hospital from June 2021 to May 2023 were selected as the study subjects,according to the random number table method,the enrolled subjects were divided into a control group and a research group,with 51 cases in each group.the control group received routine swallowing function training,the research group performed the Tongguan Liqiao acupuncture combined with electrical stimulation for swallowing disorders on the basis of the control group.Clinical efficacy,surface electromyography(sEMG)before and 30 days after treatment,and Swallowing-Quality of Life(SWAL-QOL)scores were compared between the two groups.Results The effective rate of swallowing function treatment in the research group was 96.08%,higher than 78.43%in the control group,the difference between groups was statistically significant(P﹤0.05).Before treatment,comparison of surface electromyographic activity levels in two groups of pharyngeal areas,there is no statistically significant difference between groups(P﹥0.05).However,after 30 days of treatment,the maximum amplitude value of the research group was higher than that of the control group,and the swallowing time was shorter than that of the control group,the difference between groups is statistically significant(P﹤0.05).Before treatment,comparison of SWAL-QOL scores between two groups,there is no statistically significant difference between groups(P﹥0.05).After 30 days of treatment,the SWAL-QOL score of the research group was higher than that in the control group,the difference between groups is statistically significant(P﹤0.05).Conclusion Tongguan Liqiao acupuncture combined with electrical stimulation for swallowing disorders in addition to conventional rehabilitation measures significantly improves the treatment efficacy for dysphagia in patients with ischemic stroke,enhances surface muscle function in the throat,thereby improving the quality of life.
