摘要
新型冠状病毒肺炎(简称新冠肺炎)疫情从2020年1月暴发到目前已形成常态化趋势,对住院医师规范化培训(简称住培)基地管理带来了严峻考验和挑战。新冠肺炎疫情对住培管理工作带来的挑战可归纳为7个方面:医疗工作的结构性改变;住院医师面临压力与心理健康;住院医师在临床中的感染风险;住院医师安全防护与生活保障;住培常规临床教学活动受限;临时启动的全面线上学习;基于疫情形势动态调整住培管理。培训基地需制定适应疫情常态化要求的住培管理机制,以有效应对随时可能出现的疫情再次暴发或反弹。武汉大学人民医院建立以"疫情防控优先、住院医师优先"为指导原则的应急响应机制,顺利完成了疫情期间的培训管理、培训招录、返岗复工、结业技能考核和师资培训等工作,并根据不同阶段的疫情特点及时确定住培管理重点和制定应急方案,有效保障了住院医师安全和培训工作的顺利开展,可为今后住培工作应对重大公共卫生事件提供经验借鉴。
The increasingly regular prevention and control of the COVID-19 pandemic since its outbreak in January has brought severe ordeals and new challenges to residency training management.The challenges could be summarized as changes in the nature of health care,stress and mental health of resident physicians,risk of infection during clinical practice for resident physicians,safety and wellbeing of resident physicians,limited teaching activities,temporarily activated online learning,and adjustment of training arrangements based on the severity of the epidemic.As the COVID-19 may reoccur or rebound at any time,hospitals for residency training need to develop a management mechanism that adapts to the epidemic situation.Renmin Hospital of Wuhan University has established an emergency response mechanism based on the guidelines of"prevention and control first,resident physicians first",and successfully completed training management,residency recruitment,resumption of clinical practice,final examination and faculty training during the epidemic.The priority of residency training management and emergency plans have been worked out based on the staging of the epidemic,which has guaranteed the safety of residents and completion of training,and provided experience for future residency training programs to respond to major public health events.
