摘要
目的探讨内镜下冷圈套切除术(cold snare polypectomy,CSP)对结直肠无蒂小息肉的疗效。方法选取2020年6月至2021年6月首都医科大学电力教学医院结直肠无蒂小息肉(直径5~9 mm)患者160例,根据手术方式分为CSP组和内镜下黏膜切除术(endoscopic mucosal resection,EMR)组,每组80例。比较两组手术时间、住院时间、金属夹使用数量、治疗费用、病变完整切除率、标本回收率、术中及术后迟发出血、一年后息肉复发情况。结果160例患者中,男95例,女65例,年龄36~79岁,平均(57.0±9.2)岁。CSP组手术时间、住院时间、金属夹使用数量、治疗费用、术后迟发出血发生率低于EMR组,术中出血发生率高于EMR组,差异均有统计学意义(P<0.05)。两组病变完整切除率和标本回收率均为100.00%;两组均无术后感染、穿孔发生,一年后复查肠镜均无息肉复发。结论与EMR相比,CSP在治疗结直肠无蒂小息肉的操作更简单且并发症少,可在保证息肉高完整切除率和极低复发率的情况下明显降低手术时间、住院时间及治疗费用。
Objective To explore the clinical therapeutic effect of endoscopic cold snare polypectomy(CSP)for small sessile colorectal polyps.Methods A total of 160 patients with small sessile colorectal polyps(diameter 5~9 mm)in ElectricPower Teaching Hospital,Capital Medical University from June 2020 to June 2021 was selected,and were divided into CSP group and endoscopic mucosal resection(EMR)group according to the surgical methods,with 80 cases in each group.The operation time,hospitalization time,number of metal clips used,treatment cost,complete resection rate of lesions,specimen recovery rate,delayed bleeding during and after operation and recurrence of polyps after one year were compared between the two groups.Results Among the 160 patients,95 were male and 65 were female,aged from 36 to 79 years,with an average age of(57.0±9.2)years.The operation time,hospitalization time,the number of metal clips used,the treatment cost and the incidence of postoperative delayed bleeding in CSP group were lower than those in EMR group,and the incidence of intraoperative bleeding was higher than that in EMR group,the differences were statistically significant(P<0.05).The complete resection rate and specimen recovery rate were both 100.00%in both groups.There was no postoperative infection or perforation,and no recurrence of polyps was found in the colonoscopy after one year in both groups.Conclusions Compared with EMR,CSP has simpler operation and fewer complications in the treatment of small sessile colorectal polyps,and greatly reduces the operation time,hospitalization time and treatment cost under the condition of high complete resection rate and extremely low recurrence rate.
