摘要
目的探讨疝环关闭技术在腹腔镜经腹腹膜前疝修补术治疗Ⅲ型腹股沟斜疝中的应用效果。方法选择2021年1月至2022年5月海丰县彭湃纪念医院普通外科接受腹腔镜经腹腹膜前疝修补术(TAPP)的80例Ⅲ型腹股沟斜疝患者为研究对象。按照随机数字表法分为对照组与观察组,每组40例。对照组给予常规手术方式操作,内环口不关闭;观察组采用缝合法关闭内环口。观察两组患者术后血清肿发生率、疼痛评分、手术时间、住院时间、复发率、腹股沟区慢性疼痛的情况。结果两组手术均顺利完成,均未中转为开放性术式。两组患者手术住院时间、术后第1天VAS评分、术中出血量比较,差异未见统计学意义(P>0.05)。观察组手术时间长于对照组(P<0.001)。观察组术后血清肿发生率(7.5%,3/40)低于对照组(32.5%,13/40),差异有统计学意义(χ^(2)=7.81,P<0.001)。结论疝环关闭技术可安全应用于腹腔镜下Ⅲ型腹股沟斜疝无张力修补手术,有效减少术后血清肿发生率,且不增加其他并发症。
Objective To investigate the application of hernia ring closure technique in laparoscopic transabdominal preperitoneal hernia repair for typeⅢindirect inguinal hernia.Methods Eighty patients with typeⅢindirect inguinal hernia who underwent laparoscopic transabdominal retroperitoneal hernia repair(TAPP)in the general surgery department of pengpai Memorial Hospital of Haifeng County from January 2021 to may 2022 were selected as the study subjects.According to the method of random number table,they were divided into control group and observation group,with 40 cases in each group.In the control group,the operation was performed in a conventional way,and the inner ring mouth was not closed.The observation group used suture to close the inner ring.The incidence of seroma,pain score,operation time,hospitalization time,recurrence rate and chronic pain in groin area were observed in both groups.Results The operations in both groups were successfully completed,and neither group was converted to open operation.There was no significant difference between the two groups in hospital stay,VAS score on the first day after operation and intraoperative bleeding(P>0.05).The operation time in the observation group was longer than that in the control group(P<0.001).The incidence of postoperative seroma in the observation group(7.5%,3/40)was lower than that in the control group(32.5%,13/40),and the difference was significant(χ^(2)=7.81,P<0.001).Conclusions Internal ring defect closure technique can be safely applied in laparoscopic typeⅢtension-free hernioplasty for indirect hernia,which can effectively reduce the incidence of postoperative seroma,and without increase other complications.
