摘要
目的探讨单孔和双孔腹腔镜下疝囊高位结扎术治疗效小儿腹股沟斜疝的临床效果及对患儿血清C反应蛋白(C-reactive protein,CRP)和肿瘤坏死因子α(Tumor necrosis factorα,TNF-α)水平的影响。方法将本院2019年10月-2020年10月期间收治的80例腹股沟斜疝小儿采用随机数字表法分为对照组和观察组,各40例。观察组患儿行单孔腹腔镜下疝囊高位结扎术治疗,对照组患儿行双孔腹腔镜下疝囊高位结扎术治疗。比较两组手术时间、术中出血量、切口长度、术后下床时间、住院时间、医疗费用、术后并发症发生率、术后复发率以及血清CRP、TNF-α水平。结果观察组患儿术中出血量、切口长度、术后下床时间和住院时间明显少于对照组,术后6h时的FLACC评分以及术后血清CRP和TNF-α水平均明显低于对照组(P<0.05)。两组手术时间、医疗费用、阴囊水肿发生率、复发率比较,差异无统计学意义(P>0.05)。结论与双孔腹腔镜下疝囊高位结扎术比较,单孔法治疗小儿腹股沟斜疝具有创伤小、术后恢复快、炎性反应程度和疼痛感轻等优势,值得临床推广应用。
Objective To investigate the clinical effect of high ligation of hernia sac under single-port and double-port laparoscopy in children with indirect inguinal hernia,and itsinfluences on serum C-reactive protein(CRP)and tumor necrosis factorα(TNF-α)levels in children.Methods 80 cases of children with indirect inguinal hernia in our hospital from October 2019 to October 2020 were randomly divided into control group and observation group,with 40 cases in each group.The observation group was treated with single port laparoscopic high ligation of hernia sac,and the control group was treated with double port laparoscopic high ligation of hernia sac.The operation time,intraoperative blood loss,incision length,postoperative ambulation time,hospital stay,medical expenses,postoperative complication rate,postoperative recurrence rate and serum CRP and TNF-αlevels were compared between the two groups.Results The intraoperative blood loss,incision length,postoperative ambulation time and hospital stay in the observation group were significantly less than those in the control group.The flacc score at six hours after operation and the serum CRP and TNF-αlevels in the observation group were significantly lower than those in the control group(P<0.05).There were no significant differences in operation time,medical expenses,incidence of scrotal edema and recurrence rate between the two groups(P>0.05).Conclusion Compared with double port laparoscopic high ligation of hernia sac,single hole method has advantages of less trauma,faster postoperative recovery,less inflammatory reaction and less pain,which is worthy of clinical application.
