摘要
结合中国现实,建立农村居民健康效用函数的理论模型,分析不同理念下的健康效用和医疗支出,进一步利用CFPS2016和2018年微观数据,使用基于倾向得分匹配的双重差分法,检验农村家庭燃料结构改善前后的健康水平变化,可以发现:(1)从整体层面上看,燃料结构的改善显著提升了农村居民的生理健康和心理健康水平;(2)异质性检验结果显示,农村女性、青壮年农村居民从清洁燃料中获得的健康收益更大,而且在当前阶段,相较于东部地区和西部地区,中部地区的农村居民从清洁燃料中的健康收益更加明显;(3)从经济层面上看,燃料结构的改善减少了燃料支出,有减少医疗支出的可能,说明了转变生活习惯对实现全民健康的重要性。研究表明,加强健康意识宣传、加快农村生活能源改造以及增加对农村居民健康补贴等有助于促进乡村振兴战略实施和城乡健康平等,实现全民健康。
Based on the reality of China,we firstly establish the theoretical model of the health utility function of rural residents,analyze the health utility and medical expenditure under different ideas,and explain the promotion effect of prevention oriented lifestyle on Residents’health.By further use of the micro data of CFPS 2016 and 2018,and the employment of double difference method based on propensity score matching,we test the health level changes of rural families before and after changing cooking fuel types.The results show that:(1)on the whole,the use of cleaner cooking fuel can significantly improve the physical and mental health of rural residents;(2)heterogeneity test results show that rural women and young rural residents get more health benefits from clean fuel,and in the current stage,compared with the eastern and western regions,rural residents in the central region benefit more significantly from clean fuel;(3)from the economic level,the use of clean fuel helps to reduce fuel expenditure and medical expenditure,which shows the importance of changing living habits in achieving national health.Therefore,some suggestions are put forward,such as strengthening health awareness publicity,speeding up the transformation of rural living energy and increasing health subsidies for rural residents.
