摘要
目的系统评价麦肯基疗法治疗腰椎间盘突出症(lumbar disc herniation,LDH)的疗效。方法系统检索Pubmed、web of science、Cochrane Library、中国知网、万方、维普数据库,收集麦肯基疗法治疗LDH的随机对照试验,检索时间控制在从建库至2018年11月。由两位评价员独立的对纳入研究进行资料提取和质量评估,如遇分歧,由研究小组集体表决。使用Revman5.3进行数据统计。结果共纳入11篇文献,总计846例患者。Meta分析结果显示,与对照组相比,麦肯基组视觉模拟评分量表(visual analogue scal,VAS)和简化McGill疼痛问卷(short-form McGill pain questionnaire,SF-MPQ)评分显著提高[(WMD=-1.38,95%CI:1.99^-0.77,P<0.01)、(WMD=-1.61,95%CI:-1.72^-1.50,P<0.01)];(Japanese orthopaedic association scores,JOA)和Oswestry功能障碍指数问卷表(Oswestry dability index,ODI)评分改善更显著[(WMD=4.20,95%CI:2.83~5.56,P<0.01)、(WMD=-5.72,95CI%:-6.96^-4.50,P<0.01)];麦肯基疗法的有效率更高(RR=1.20,95%CI:1.12~1.30,P<0.01),复发率更低(RR=0.34,95%CI:0.15~0.76,P=0.008)。结论麦肯基疗法可以有效缓解LDH患者腰部疼痛,恢复或重建腰部功能,提高患者的生活质量。
Objective To systematically evaluate the effect of Mckenzie therapy on patients with lumbar disc herniation(LDH).Methods Pubmed,Web of science,Cochrane Library,Chinese national knowledge infrastructure(CNKI),Wan fang,VIP were searched,and the randomized controlled trials of McKenzie therapy for LDH were collected.The retrieval time was controlled from the establishment of the database to November 2018.Two judges independently extracted data and conducted quality assessment of the included studies.In case of disagreement,the study group voted collectively.Data statistics was conducted by RevMan 5.3.Results A total of 11 studies were included,with 846 patients.The results of Meta-analysis showed that Mckenzie therapy was better than the control group in the improvement of visual analogue scal(VAS)score and short-form McGill pain questionnaire(SF-MPQ)score[(WMD=-1.38,95 CI:1.99–-0.77,P<0.01),(WMD=-1.61,95%CI:-1.72–-1.50,P<0.01)].Mckenzie therapy was better than the control group in the improvement of Japanese orthopaedic association scores(JOA)and Oswestry dability index(ODI)score[(WMD=4.20,95%CI:2.83–5.56,P<0.01),(WMD=-5.72,95 CI:-6.96–-4.50,P<0.01)].The effective rate of Mckenzie therapy was higher than the control group(RR=1.20,95%CI:1.12-1.30,P<0.01),the recurrence rate of Mckenzie therapy was lower than the control group(RR=0.34,95%CI:0.15-0.76,P=0.008).Conclusion McKinkey therapy can effectively relieve lumbar pain on patients with LDH,restore or reconstruct lumbar function,and improve the quality of life of patients.
