摘要
目的分析2018年11月深圳市大鹏新区某小学1起呼吸道感染疫情的病原学及分子遗传学特征。方法采用实时荧光RT-PCR对采集的患儿咽拭子标本进行常见呼吸道病毒及肠道病毒的检测,对肠道病毒阳性样本进一步分型鉴定。通过RT-PCR扩增、测序并获得VP1基因序列,运用MEGA 7.0软件分析VP1的分子特征,构建遗传进化树。结果采集的11份样本全部是常见呼吸道病毒阴性,其中5份为柯萨奇病毒A组(简称CVA4)阳性。VP1区基因序列分析表明:本次疫情毒株的核苷酸和氨基酸同源性分别为99.9%~100.0%,100.0%。系统进化树结果显示,5株毒株亲缘关系最近,应来自于同一亲缘株;与国内分离株位于同一分支,而且与2017年山东、2016年广东和宁夏的分离株亲缘关系较近;5株毒株与国外的CVA4毒株位于不同的分支,亲缘关系较远,其中包括来自美国的CVA4原型株AY421762,但未见特异性的氨基酸差异位点。结论本起呼吸道感染暴发疫情由CVA4毒株引起,肠道病毒引起的呼吸道系统疾病应受到足够重视。
Objective To determine the pathogen and moleculargenetic characteristics of a respiratory infection outbreak caused by CVA4 in aprimary school.Methods The throat swab samples collected from childrenwere detected for the common respiratory viruses andenterovirus,and the genotype ofenterovirus-positive samples wasfurther identified.RT-PCR and sequencing were used to identify the sequences of VP1 genes;MEGA 7.0 software was employed to analyze the molecular characteristics of VP1 gene and construct geneticphylogenetic tree.Results All the collectedsamples were negative for common respiratory viruses,but5 of them were positive for Coxsackievirus A4(CVA4).The sequences of VP1 gene showed that the homologies of nucleotide and amino acid among the 5 isolateswere 99.9%-100.0%,100.0%,respectively.The phylogenetic tree indicated that the 5 isolates which had the closest relationship could be derived from a same original source.The 5 CVA4 isolates were clustered in the same subclade with those collected from other regions of China.The relationship between 5 isolates in this study wasclosetothe isolates identified from Shandong in 2017,Guangdong in 2016,Ningxia in 2016.The 5 isolates were not clustered in the samesub-clades with isolates identified from other countries like the CVA4 isolate AY421762 from the USA,indicating a distant relationship between these isolates,but none of special mutation sites of amino acid were found.Conclusion This studied case of respiratory infection outbreak was caused by CVA4,and the respiratory diseases caused by enterovirus should be paid sufficient attention to.
