摘要
目的:探讨异丙托溴铵联合布地奈德福莫特罗对哮喘-慢阻肺重叠(ACOS)患者血清炎症因子及肺功能的影响。方法:将60例ACOS患者随机分为3组每组20例,分别采用布地奈德福莫特罗、异丙托溴铵以及二者联合治疗6个月。比较治疗前后3组患者血清CRP、IL-4、IL-6和TNF-α水平,肺功能指标,以及SCRQ生活质量。结果:治疗后各组CRP、IL-4、IL-6、TNF-α及SCRQ评分下降,肺功能改善;治疗6个月后联合组IL-6、TNF-α低于其他两组;治疗3、6个月后联合组肺功能改善程度高于其他两组;治疗3个月后联合组SCRQ评分低于异丙托溴铵组,而与布地奈德福莫特罗无差异,治疗6个月后联合组SCRQ评分均低于其他两组。结论:异丙托溴铵联合布地奈德福莫特罗治疗ACOS的效果较好,能降低CRP、IL-4、IL-6、TNF-α,改善肺功能,提高生活质量,值得推广应用。
Objective To investigate the effect of ipratropium bromide combined with budesonide formoterol on serum inflammatory factors and lung function in patients with asthma chronic obstructive pulmonary overlap syndrome(ACOS).Methods A total of 60 patients with ACOS were randomly divided into 3 groups with 20 cases in each group,and treated with budesonide formoterol,ipratropium bromide and combined medication.The course of treatment was 6 months.The levels of CRP,IL-4,IL-6 and TNF-α,lung function indexes and SCRQ quality of life were compared before and after treatment.Results After treatment,the levels of CRP,IL-4,IL-6,TNF-αand scores of SCRQ were decreased and the lung function was improved.The IL-6,TNF-αin the combined group were lower than that of the other two groups at 6 months after treatment,and the improvement of lung function in the combined group was higher than that of the other two groups at 3 and 6 months after treatment.After 3 months of treatment,the scores of SCRQ in the combined group were lower than those of the ipratropium bromide group,but there was no difference with budesonide.The scores of SCRQ in the combined group were lower than those of the other two groups at 6 months after treatment.Conclusion Ipratropium bromide combined with Budesonide Formoterol in the treatment of ACOS is better.It can reduce CRP,IL-4,IL-6,TNF-α,improve lung function and improve the quality of life.It is worth popularizing.
