摘要
目的:探究微卫星不稳定性与结直肠癌的免疫微环境的关系。方法:根据微卫星不稳定性,将316例结直肠癌患者分为MSI-H组与MSS/MSI-L组,采用免疫组织化学染色法检测两组中4种错配修复(MMR)蛋白的表达情况。从GEO数据库下载结直肠癌数据集GSE24551,其包括147例结直肠癌患者转录组表达谱、临床病理资料以及MSI检测结果。利用临床资料分析MSI与结直肠癌的临床病理特征的关系,利用GEO数据库分析MSI与结直肠癌患者免疫检查点表达、免疫浸润以及预后的关系。结果:与MSS/MSI-L组比较,MSI-H组具有多位于右半结肠、直径大于5cm、组织成分中含有黏液的特征,有统计学差异。两组无病生存期有统计学差异。MSI-H组具有更高的PD-L1转录组表达及不同的免疫细胞浸润模式。结论:MSI-H型CRC具有特殊的临床病理特征,且预后更好。不同MSI状态下,CRC免疫微环境有差异。对于CRC患者进行MSI筛查,有助于免疫治疗方案的制定。
Objective To explore the relationships between Microsatellite Instability and Immune Microenvironment in Colorectal Cancer.Methods 316 CRC were classsfied as MSI-H/MSI-L according to MSI status determined by immunostaining of 4 Mismatch Repair(MMR)proteins.GSE24551 dataset was downloaded from the Gene Expression Omnibus(GEO)database,which contained transcriptomic expression profiles,clinical data,and MSI results of 147 CRC patients.The relationship between MSI and clinicopathological characteristics of CRC was discovered by analyzing immunohistochemical staining data.The relationship between MSI and Immune Check Point expression patterns,Immune Infiltration patterns,and prognosis of CRC was discovered by conducting bioinformatics analysis on the GSE24551 dataset.Results Compared with the MSS/MSI-L group,the MSI-H group has the characteristics of mostly located in the right hemicolectomy,with a diameter greater than 5 cm,and tissue components containing mucus,which is statistically different.There was a statistical difference in Disease-Free Survival(DFS)between the two groups.The MSI-H group had a higher PD-L1 expression level and possessed a distinctive immune infiltration pattern.Conclusion CRC patients with MSI-H status have unique clinicopathological features and a better DFS prognosis result.Patients with MSI-H and MSS/MSI-L status have distinctive immune infiltration patterns and thus MSI screening prior to treatment is beneficial for immunotherapy planning.
