摘要
以2015年中国健康与养老追踪调查数据(CHARLS)为基础,构建结构方程模型分析基本医疗保险制度与老龄健康公平之间的关系,实证结果表明,年龄、配偶与收入是导致老龄健康不公平的重要因素。基本医疗保险制度促进了老龄群体的门诊医疗服务利用公平性,但未能改善住院医疗服务利用的不公平现象;城镇职工基本医疗保险与城镇居民基本医疗保险显著促进了老龄群体的医疗卫生服务利用公平,对于提高老龄群体健康水平具有积极作用,而新型农村合作医疗的政策效果则不明显。建议合理分配城乡医疗资源,加快推进基本医疗保险城乡统筹,同时鼓励和支持家庭承担老年照料责任,以提升老龄健康水平、促进老龄健康公平。
Based on data from the 2015 China Health and Retirement Longitudinal Study(CHARLS),the present study constructed a structural equation model to analyze the relationship between the basic medical insurance system and health equity among elderly persons.The empirical results showed that age,spouse,and income were important factors leading to health inequities among the elderly.The basic medical insurance system was found to have promoted equity in the utilization of outpatient medical services in the elderly population but fail to reduce inequity in the utilization of inpatient medical services.Basic medical insurance for both urban employees and urban residents had significantly promoted equity in the utilization of medical and health care services in the elderly population and played a positive role in improving their health;however,the new rural cooperative medical scheme had no significant effect on these areas.It is recommended that the government allocate medical resources to urban and rural areas in a rational manner and speed up the coordination of basic medical insurance for urban and rural areas,while encouraging and supporting families to take care of elderly persons,to improve their health and promote health equity among them.
