摘要
目的分析流感患者的症状及血液学特点。方法选择456例流感患者和487名健康体检者,采用血细胞分析仪对白细胞(WBC)进行计数和分类,分析WBC计数、中性粒细胞计数及百分比、淋巴细胞计数及百分比、单核细胞计数及百分比、嗜碱性粒细胞计数及百分比。观察各项指标的灵敏度、特异度、约登(Youden)指数和受试者工作特征(ROC)曲线下面积以评价其诊断价值。结果绝大多数流感患者有发热、浑身酸痛及呼吸道症状;流感组与健康对照组血常规各项观察指标组间比较,差异均有统计学意义(P<0.05)。淋巴细胞计数的ROC曲线面积为0.935,诊断准确性较高。嗜碱性粒细胞百分比的Youden指数为0.78,诊断效果较好。结论流感早期,患者的临床症状主要为发热伴呼吸道症状及浑身酸痛,血常规变化主要是淋巴细胞降低、单核细胞增高、嗜碱性粒细胞增高、C-反应蛋白(CRP)正常或轻度升高,而WBC总数及中性粒细胞数则基本正常。
Objective To analyze the symptoms and blood characteristics of influenza patients.Methods 456 cases of influenza patients and 487 healthy people were selected and the blood cell analyzer was used to count and classify the White blood cell(WBC).WBC,neutrophil,lymphocyte,mononuclear cells,basophilic cells count and percentage were analyzed.Diagnostic test methods such as sensitivity,specificity,Youden index and area under the receiver operating characteristic(ROC)curve were observed to evaluate their diagnostic value.Results Most patients had fever,body ache andrespiratorysymptoms.There were statistically significant differences between the influenza group and the healthy control group(P<0.05).The area under the ROC curve of lymphocyte count was 0.935,indicating high diagnostic accuracy.The Youden index of basophilic cell percentage was 0.78,indicating a good diagnostic effect.Conclusion In the early stage of influenza,the clinical symptoms of the patients were mainly fever with respiratory symptoms and body ache.Blood routine changes were mainly lymphocyte decrease,monocytes increase,basophilic cells increase,normal or slightly increased CRP,while the total numbers of WBC and neutrophils were basically normal.
