摘要
目的应用Meta分析方法评价以血流储备分数(FFR)为参考,磁共振心肌灌注成像(MR-MPI)在诊断冠心病准确性中的价值。方法检索以FFR诊断为金标准,MR-MPI诊断冠心病的文献,提取符合纳入标准的诊断试验数据。运用Meta-disc1.4和STATA12.0软件进行统计分析:方法包括异质性检验、发表偏倚检测等,合并汇总敏感性(SEN)、特异性(SPE)、95%可信区间(95%CI),绘制出受试者工作特征(SROC)曲线同时计算其曲线下面积(AUC)。结果本研究最终纳入文献24篇,评价了1529例患者和4535支血管。在患者水平汇总的敏感性和特异性分别为0.88(0.85,0.90)、0.87(0.84,0.89),在血管水平汇总的敏感性和特异性分别为0.81(0.78,0.83)、0.89(0.88,0.90),患者水平、血管水平对应的AUC分别为0.93、0.92。结论以FFR为参考,MR-MPI在诊断冠心病时敏感度及特异度较高,诊断能力强。
Objective To evaluate the value of magnetic resonance myocardial perfusion imaging(MR-MPI)in diagnosing coronary artery disease by using the method of Meta-analysis,with the reference of blood flow reserve fraction(FFR).Methods The literature on the diagnosis of coronary heart disease by MR-MPI was performed by using the blood flow reserve fraction(FFR)as the gold standard,and the diagnostic test data meeting the inclusion criteria was extracted.Statistical analysis was performed using Meta-disc1.4 and STATA12.0 software:including heterogeneity test,publication bias test,combined sensitivity(SEN),specificity(SPE)and 95%confidence interval(95%CI).The receiver operating characteristic curve(SROC)was plotted and the area under the curve(AUC)was calculated.Results The study finally included 24 articles,1529 patients and 4535 blood vessels were evaluated.The sensitivity and specificity at the patient level were 0.88(0.85,0.90)and 0.87(0.84,0.89),respectively.The sensitivity and specificity at the vascular level were 0.81(0.78,0.83)and 0.89(0.88,0.90,respectively).The AUC of the patient's level and blood vessel level were 0.93 and 0.92,respectively.Conclusion Based on FFR,MR-MPI has high sensitivity and specificity in the diagnosis of coronary heart disease,and its diagnostic ability is strong.
