摘要
目的对MRI在结直肠癌肝转移病变中的诊断价值进行Meta分析。方法检索自建库起至2020年4月12日PubMed、EMBASE、Cochrane、CNKI、万方数据库、中国生物医学文献数据库(CBM)中有关MRI诊断结直肠癌肝转移的相关文献。按照诊断试验的纳入和排除标准筛选文献,运用QUADAS-2文献质量评价工具及Review Manager 5.3软件评价纳入文献的质量、偏倚风险,采用Stata 12.0软件进行Meta分析。结果最终纳入17篇文献,其中英文16篇,中文1篇,Meta分析结果显示,MRI鉴别结直肠癌良恶性病变的合并敏感性(Sen合并)为0.90(95%CI,0.80~0.95),特异性(Spe合并)为0.90(95%CI,0.82~0.95),阳性似然比(+LR合并)为9.09(95%CI,4.97~16.63),阴性似然比(-LR合并)为0.12(95%CI,0.06~0.22),诊断比值比(DOR合并)为78.55(95%CI,34.13~180.79);SROC曲线下面积为0.94、Q*为0.88。结论MRI对结直肠癌肝转移具有较高的诊断价值。
Objective To analyze the value of MR in the diagnosis of liver metastasis of colorectal cancer.Methods From the establishment of the database to March 12,2020,PubMed,EMBASE,Cochrane,Wanfang database,China biomedical literature database,and China Journal of science and technology database were searched for the relevant literature on MRI diagnosis of liver metastasis of colorectal cancer.According to the inclusion and exclusion criteria of diagnostic tests,literature quality assessment tool quadas-2 and Review Manager 5.3 software were used to evaluate the bias risk of the included literature,and meta analysis was conducted by Using Stata 12.0 software.Results Ultimately,17 articles were included,comprising of 16 articles in English and 1 in Chinese.The results of meta analysis showed that,the pooled sensitivity(Sen),specificity(Spe),positivelikelihood ratio(+LR),negative likelihood ratio(-LR),diagnostic odds ratio(DOR)of MRI for differentiating liver metastasis of colorectal cancer lesions were 0.90(95%CI,0.80~0.95),0.90(95%CI,0.82~0.95),9.09(95%CI,4.97~16.63)0.12(95%CI,0.06~0.22),78.55(95%CI,34.13~180.79);the area under the summary receiver operating characteristic(SROC)curve was 0.94,Q*was 0.88.Conclusion MR imaging has high diagnostic value for liver metastasis of colorectal cancer.
