光谱CT虚拟平扫在肝脏及肾上腺CT衰减一致性研究 被引量:13

Study on CT Attenuation Consistency of Liver and Adrenal Glands by Spectral CT Virtual Non-Contrast Images

摘要 目的探讨IQon光谱CT虚拟平扫(VNC)与常规平扫(TNC)在肝脏及肾上腺CT衰减的一致性。方法搜集2019年4月至2019年9月南京大学医学院附属鼓楼医院腹部光谱CT增强图像50例患者,包含平扫、动脉期及静脉期图像。将动静脉期光谱图像导入工作站,分别生成VNC图像。在同一层面的TNC和VNC图像上,勾画肝脏和肾上腺相同部位的圆形感兴趣区(ROI),以测量CT值。采用正态分布检验结果,差异性检验采用非参数两相关样本检验。TNC和VNC CT值的一致性通过组内相关系数(ICC)评估。结果双侧肾上腺ROI的动、静脉期VNC和TNC CT值均存在显著性差异(P值均<0.05)。而肝脏ROI的动脉期VNC与TNC CT值均存在显著性差异(P值均<0.05),静脉期的VNC与TNC CT值均无显著性差异(P值均>0.05)。肝脏ROI的静脉期VNC与TNC CT值ICC>0.800,其中较大面积ROI的ICC值最高,为0.908。结论基于IQon光谱CT所获得的腹部较大器官,如肝脏增强扫描静脉期的VNC在CT值测量中可以替代TNC,两者一致性较高。但在腹部较小器官,如肾上腺中,其VNC的价值有待进一步验证。 Objective To investigate the computed tomography(CT)attenuation consistency between virtual non-contrast(VNC)derived from spectral CT and true non-contrast CT(TNC)in liver and adrenal glands.Methods Fifty cases of abdominal contrast enhancement CT images(including non-contrast,arterial and venous phases)performed from April 2019 to September 2019 at Nanjing Drum Tower Hospital were retrospectively analyzed.The arterial and venous spectral images were imported into the workstation to generate VNC images.CT values of the same part were obtained via a delineating circular region of interest(ROI)on the same level of TNC and VNC images of the liver and adrenal glands.According to the results of the normal distribution test,a non-parametric two-related sample test was used to assess difference.The consistency of CT values between TNC and VNC was assessed by intraclass correlation coefficient(ICC).Results There were significant differences of CT values between VNC and TNC on both sides of the adrenal in arterial and venous phases(P<0.05).There were significant differences of CT values between VNC and TNC in arterial phase on liver(P<0.05).There was no significant difference of CT values between VNC and TNC in venous phase(P>0.05).The ICCs between the CT values of VNC and TNC in the venous phase on liver were higher than 0.800,especially for the large ROI(ICC=0.908).Conclusion The venous VNC images based on IQon spectral CT for large organs like liver may replace TNC in CT measurement,and the ICCs between them were high,But the use of VNC in small organs like adrenal glands is yet to be further verified.
