摘要
目的基于现代临床文献探讨中医药治疗产后抑郁症(postpartum depression,PPD)内服方药的药物组成规律及遣方用药思路。方法检索中国知网、万方数据库等国内中文数据库2000—2019年发表的关于中药治疗PPD临床研究的相关文献,根据纳入及排除标准选择研究所需的中药内服方剂。建立数据库,采用SPSS 24.0、Weka 3.8软件对中药进行频数频率、聚类及关联规则分析,探索中药治疗PPD的遣方用药规律。结果共筛选处方112例,涉及中药116味。出现频数≥处方率15%的高频药物有17味,前6味分别是柴胡、甘草、当归、白芍、茯苓、白术。所有药物中,归经以脾经、肝经为主,温性和甘味药出现频率最高。药物功效以补益类、理气类、安神类、活血化瘀类等为主。中医治疗PPD的中药组合有具有疏肝解郁,健脾和中,益气补血,理气活血、养心安神等功效特点。治疗PPD的中药药对以疏肝、健脾、补气、养血等功效为主。结论中医药临床治疗PPD注重调理气血,灵活运用疏肝解郁、理气健脾、益气养血、养心安神等治法,重视以逍遥散为基础方的化裁应用。
Objective To explore the drug composition rule of traditional Chinese medicine in the treatment of postpartum depression(PPD)and the law of medicine based on the modern clinical literature.Methods We searched the related literature of clinical research on PPD treated by traditional Chinese medicine published by CNKI,Wanfang database and other domestic Chinese databases from 2000 to 2019,and selected the prescriptions of traditional Chinese medicine for internal use according to the inclusion and exclusion criteria.The database was established,and SPSS 24.0 and Weka 3.8 software were used to analyze the frequency,clustering and association rules of traditional Chinese medicine,and the prescription and medication rules of traditional Chinese medicine in the treatment of PPD were explored.Results A total of 112 prescriptions were screened,involving 116 kinds of traditional Chinese medicine.There were 17 high-frequency drugs with frequency≥15%of the prescription rate,and the first six were Chaihu(Bupleuri Radix),Gancao(Glycyrrhizae Radix Et Rhizoma),Danggui(Angelicae Sinensis Radix),Baishao(Paeoniae Radix Alba),Fuling(Poria)and Baizhu(Atractylodis Macrocephalae Rhizoma).Among all the drugs,the spleen and liver meridians were the main meridians,and the warm and sweet drugs had the highest frequency.The main functions of the drugs were tonifying,regulating Qi,calming nerves,promoting blood circulation and removing blood stasis.The combination of traditional Chinese medicine in the treatment of PPD had the characteristics of soothing liver and relieving depression,strengthening spleen and the middle,supplementing Qi and blood,regulating Qi and activating blood,nourishing heart and tranquilizing the mind.The main functions of traditional Chinese medicine in the treatment of PPD were to soothe liver,strengthen spleen,replenish Qi and nourish blood.Conclusion In the clinical treatment of PPD,traditional Chinese medicine pays attention to the regulation of Qi and blood,flexibly applies the treatment methods such as soothing liver and relieving depression,regulating Qi and strengthening spleen,benefiting Qi and nourishing blood,nourishing heart and tranquilizing the mind and pays attention to the application of Xiaoyao Powder as the basis.
