摘要
目的探究自拟化痰开肺方治疗闭塞性细支气管炎痰瘀蕴肺证的临床疗效。方法选取2018年10月—2019年12月黑龙江中医药大学附属第一医院收治的闭塞性细支气管炎(痰瘀蕴肺证)患儿60例,随机数表法分为观察组(33例)和对照组(27例)。对照组给予阿奇霉素治疗,观察组在此基础上加服自拟化痰开肺方。比较两组患儿的临床疗效,临床症状体征消失时间,治疗前后血清炎性因子和T淋巴细胞亚群水平变化。结果观察组的治疗总有效率为90.91%(30/33),高于对照组的66.67%(18/27)(P<0.05);观察组患儿的咳嗽消失时间、气促消失时间和肺部啰音消失时间均短于对照组患儿(P<0.05);治疗后,两组患儿的血清白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)和CD^(+)_(8)水平均明显下降,CD^(+)_(4)水平明显升高(P<0.05),且观察组的血清IL-8、TNF-α、IFN-γ和CD^(+)_(8)水平低于对照组,CD^(+)_(4)水平高于对照组(P<0.05)。结论自拟化痰开肺方在治疗痰瘀蕴肺证闭塞性细支气管炎方面疗效确切,能有效改善患儿的临床症状,降低血清炎性因子的表达水平,提高免疫功能,促进患儿恢复。
Objective To explore the clinical curative effect of self-made Huatan Kaifei Prescription(化痰开肺方)on bronchiolitis obliterans(BO)of phlegm-blood stasis accumulating in lung syndrome.Methods A total of 60 BO children(phlegm-blood stasis accumulating in lung syndrome)who were admitted to the hospital from October 2018 to December 2019 were enrolled and divided into the observation group(33 cases)and the control group(27 cases)by the random number table method.The control group was treated with azithromycin,while the observation group was treated with self-made Huatan Kaifei Prescription on the basis of the control group.The clinical curative effect,disappearance time of clinical symptoms and signs and changes in levels of serum inflammatory factors and T lymphocyte subsets before and after treatment were compared between the two groups.Results The total response rate of treatment in the observation group was higher than that in the control group[90.91%(30/33)vs 66.67%(18/27)](P<0.05).The disappearance time of cough,anhelation and lung rales in the observation group were shorter than those in the control group(P<0.05).After treatment,the levels of serum interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ)and CD^(+)_(8)were significantly decreased,while the CD^(+)_(4)level was significantly increased in both groups(P<0.05).The levels of serum IL-8,TNF-α,IFN-γand CD^(+)_(8)in the observation group were lower than those in the control group,while the CD^(+)_(4)level was higher than that in the control group(P<0.05).Conclusion The curative effect of self-made Huatan Kaifei Prescription is significant on bronchiolitis obliterans of phlegm-blood stasis accumulating in lung syndrome.It can effectively improve the clinical symptoms of children,reduce the expression levels of serum inflammatory factors,improve the immune function and promote their recovery.
