摘要
目的:探讨乳腺术后三阴方对三阴性乳腺癌患者临床症状、ECOG、KPS评分干预情况,聚类归纳分析三阴性乳腺癌中医致病因素,为中药治疗三阴性乳腺癌提供新思路。方法:收集2017年1月—2017年9月在上海中医药大学附属龙华医院门诊就诊及复旦大学附属肿瘤医院住院手术后化疗患者,均经病理诊断为三阴性乳腺癌共60例,根据前瞻性同期病例对照研究模式,按研究中心分为试验组(龙华医院)与对照组(复旦大学附属肿瘤医院),每组各30例,试验组予乳腺术后三阴方加减治疗,对照组仅西药治疗,随访6个月,观察随访前后患者临床症状积分及KPS、ECOG评分情况。结果:(1)随访6个月后,试验组各症状较对照组皆有不同程度改善(P<0.05),症状改善总有效率达到87%较对照组差异有统计学意义(P<0.05)。(2)将纳入患者中医症状进行聚类分析后可归为6类,进而总结三阴性乳腺癌术后病理因素为:湿、虚、郁、瘀、痰、风,且各病理因素之间互相胶结共发为病。(3)随访6个月后,两组患者体力状态均较前有所好转,试验组健康状况恢复的更快,程度恢复的更大,其中KPS评分差异具有统计学意义(P<0.05),试验组好转人数较对照组显著增多(P<0.05)。(4)两组在治疗随访期间无明显不良事件发生,纳入患者随访期间均未出现肿瘤复发及转移征象。结论:乳腺术后三阴方对三阴性乳腺癌症状改善疗效确切,且组方切合疾病特点,为中医药治疗三阴性乳腺癌细化方向,值得进行进一步研究。
Objective:To explore the intervention of Sanyin Decoction on clinical symptoms,ECOG and KPS scores of patients with triple negative breast cancer,and to cluster and summarize the pathogenic factors of triple negative breast cancer in TCM,so as to provide new ideas for TCM treatment of triple negative breast cancer.Method:From January 2017 to September 2017,60 patients with triple negative breast cancer were diagnosed by pathology in the outpatient clinic of Longhua Hospital affiliated to Shanghai University of Traditional Chinese Medicine and in the post-operative chemotherapy of Tumor Hospital affiliated to Fudan University.According to the prospective case-control study model,the patients were divided into experimental group(Longhua Hospital)and control group(Tumor Hospital affiliated to Fudan University).The experimental group was treated with Sanyin Decoction,while the control group was treated with western medicine only.30 patients in each group were followed up for 6 months.The clinical symptom scores,KPS and ECOG scores were observed before and after the follow-up.Results:(1)After 6 months of follow-up,the symptoms of the experimental group were improved to different degrees compared with the control group(P<0.05).The total effective rate of symptoms improvement reached 87%compared with the control group(P<0.05).(2)After clustering analysis,the patients with TCM symptoms could be classified into six categories.The pathological factors after operation of the three negative breast cancer were summarized as follows:dampness,deficiency,depression,blood stasis,phlegm,wind,and various diseases.Physical factors cemented with each other and co-occured as diseases.(3)After 6 months of follow-up,the physical condition of the two groups improved.The health condition of the experimental group recovered faster and the degree of recovery was greater.The KPS score had statistical significance(P<0.05).The number of improvement in the experimental group was significantly higher than that in the control group(P<0.05).(4)There were no significant adverse events in the two groups during the treatment follow-up period.There were no signs of recurrence or metastasis during follow-up.Conclusion:After breast surgery,Sanyin prescription has a definite effect on improving the symptoms of triple negative breast cancer,and the prescription is in line with the characteristics of the disease.It is a refined direction for the treatment of triple negative breast cancer by traditional Chinese medicine,which deserves further study.
