摘要
目的原发性肝癌(PLC)恶性程度高,侵袭转移力强,症状隐匿,缺乏典型临床表现,作为液体活检代表的循环肿瘤细胞(CTCs)能否助力肝癌的诊治是当前探讨的热点问题。方法检索中国知网、万方、PubMed数据库自建库至2022年1月发表的相关文献,以“肝癌(liver cancer)”或“肝细胞癌(hepatocellular carcinoma)”“循环肿瘤细胞(CTCs)”“液体活检(liquid biopsy)”为关键词,纳入标准:肝癌CTCs检测;肝细胞癌CTCs检测;肝癌液体活检相关研究。排除标准:非肝细胞癌、循环肿瘤DNA等非相关文献,最终纳入43篇文献。结果CTCs能够较好地反映肿瘤异质性和整体情况,在转移过程中可能会发生上皮-间质转化(EMT),上皮细胞标志物表达下调,间充质细胞标志物表达上调,导致形态改变并重新获得增殖能力且具有很高的侵袭性,对肝癌患者的诊断和预后评价有着重要临床意义。肝癌中CTCs的研究主要采用基于免疫磁珠技术开发的CellSearch系统、膜过滤方法、流式细胞分析和传统的免疫磁珠富集法。结论外周血CTCs检测可能为肝癌患者诊断、预后疗效评价提供参考价值,液体活检凭借非侵入性、取样方便、可动态监测等优势,因此CTCs已成为肝癌诊治研究领域的热点方向。
Objective Primary liver cancer(PLC)is characterized by high malignancy,strong invasion and metastasis,insidious symptoms,and lack of typical clinical manifestations.The application of circulating tumor cells(CTCs),as a representative of liquid biopsy and possibility to help the diagnosis and treatment of liver cancer are hot topics in the current discussion.Methods The related literatures published from the establishment of the database to January 2022 were retrieved from China National Knowledge Infrastructure(CNKI),Wanfang and PubMed databases,with"liver cancer"or"hepatocellular carcinoma","CTCs"and"liquid biopsy"as the key words.Inclusion criteria:detection of CTCs liver cancer CTCs;detection of CTCS in hepatocellular carcinoma;studies on liquid biopsy of liver cancer.Exclusion criteria:non-related literatures such as non-hepatocellular carcinoma and circulating tumor DNA.Finally,43 articles were included in the analysis.Results CTCs can better reflect the heterogeneity and overall situation of tumors.It may occur EMT,down-regulated expression of epithelial cell markers,up-regulated expression of mesenchymal cell markers,resulting in morphological changes and regaining proliferation ability.CTCs are highly invasive and have important clinical significance for the diagnosis and prognosis evaluation of liver cancer patients.The study of CTCs in liver cancer mainly adopts cellsearch system based on immunomagnetic bead technology,membrane filtration method,flow cytometry and traditional immunomagnetic bead enrichment method.Conclusion The detection of peripheral blood CTCs may provide reference value for the diagnosis and prognosis evaluation of liver cancer patients.The advantages of convenient sampling and dynamic monitoring,therefore CTCs have become a hot topic in the field of diagnosis and treatment of liver cancer.
