The prognostic value of epstein-barr virus antibodies in nasopharyngeal carcinoma

摘要 目的探讨EB病毒衣壳抗原(VCA)-IgA和核抗原1(NA1)-IgA抗体在鼻咽癌中的风险预估价值。方法选取医院住院部未进行治疗的鼻咽癌病人和鼻咽癌筛查人群,抽取静脉血,运用酶联免疫方法进行EBV-VCA-IgA和EBV-NA1-IgA抗体检测,运用ROC曲线和二元logistic回归分析对数据进行统计分析。结果鼻咽癌组中VCA-IgA和NA1-IgA抗体分布与正常对照组差异有统计学意义(P<0.05)。ROC曲线分析显示,VCA-IgA检测曲线下面积最大(AUC=0.919),其cut-off值设为0.493时敏感度最高,达到91%,当与NA1-IgA进行联合检测时,特异度可以达到100%。二元Logistic回归分析显示,当VCA-IgA S/CO值为0.493~0.915,其得鼻咽癌的风险比<0.493时高9.948倍(P=0.018),95%置信区间为1.473~67.161。当VCA-IgA为≥0.915,其得鼻咽癌的风险比<0.493时高46.978倍(P=0.001),95%置信区间为9.393~234.953。当NA1-IgA S/CO值为0.594~2.436,其得鼻咽癌的风险比<0.594时高6.915倍(P=0.031),95%置信区间为1.199~39.882。当NA1-IgA为≥2.436,其得鼻咽癌的风险比<0.594时高9.644倍(P=0.012),95%置信区间为1.634~56.931。结论EB病毒VCA-IgA和NA1-IgA抗体值升高处于不同范围时,其鼻咽癌风险系数相应增加。 Objective To explore the risk assessment value of epstein-barr virus(EBV)capsid antigen IgA(VCA-IgA)and nuclear antigen 1 IgA(NA1-IgA)in the nasopharyngeal carcinoma(NPC).Methods The NPC patients not treated in hospital and the people participated in the NPC screening project were recruited in this study.The venous blood was extracted to test the EBV-VCA-IgA and EBV-NA1-IgA using ELISA method.The ROC curve and binary logistic regression were adopted to analyze the data.Results There were significant differences between the NPC group and control group on the distribution of VCA-IgA and NAl-IgA(P<0.05).The analysis of ROC curve showed that the area under the ROC curve was the biggest(AUC=0.919).When the cut-off value was set to 0.493,the sensitivity was the highest and reached 91%.If the VCA-IgA and NAl-IgA were combined,the specificity could be raised to 100%.The results of logistic regression indicated that when the S/CO value of VCA-IgA was 0.493-0.915,the risk of NPC was 9.948 times higher than that of<0.493(P=0.018).The 95%confidence was 1.473-67.161.If the value of VCA-IgA was≥0.915,the risk was 46.978 times higher than that of<0.493(P=0.001).The 95%confidence was 9.393-234.953.When the value of NA1-IgA was 0.594-2.436,the risk was 6.915 times higher than that of<0.594(P=0.031).The 95%confidence was 1.199-39.882.When the value of NAl-IgA was≥2.436,the risk was 9.644 times higher than that of<0.594(P=0.012).The 95%confidence was1.634-56.931.Conclusion The risk of NPC was increased according to different ranges of EBV VCA-IgA and NA1-IgA values.
