缺铁性贫血患者感染风险与基线铁状态及其代谢的关系 被引量:4

Relationship between infection risk and baseline iron status, metabolism in patients with iron deficiency anemia

摘要 目的探讨缺铁性贫血患者感染风险与基线铁状态及其代谢的关系。方法选取2017年5月-2020年6月南京医科大学附属江宁医院收治的50例缺铁性贫血合并感染患者作为感染组,再选取54例同期单纯缺铁性贫血患者作为非感染组,比较两组患者相关资料,并探讨缺铁性贫血患者感染与铁代谢指标之间的关系。结果感染组血清铁、铁蛋白、转铁蛋白饱和度水平均低于非感染组,总铁结合力水平高于非感染组,差异有统计学意义(t=3.098、15.748、2.449、3.573,P均<0.05);两组白细胞计数、中性粒细胞计数比较差异有统计学意义(χ^(2)=4.517、7.429,P均<0.05);多因素logistic回归分析显示血清铁、铁蛋白、转铁蛋白饱和度、中性粒细胞计数是缺铁性贫血患者感染发生的独立危险因素(P=0.001、<0.001、<0.001、<0.001);相关性分析显示,血清铁、铁蛋白、转铁蛋白饱和度与中性粒细胞计数成负相关(r=-0.534、-0.609、-0.650,P均<0.05)。结论缺铁性贫血患者感染发生与铁代谢相关指标密切相关,临床可针对上述危险因素制定治疗方案以预防感染发生。 Objective To explore the relationship between infection risk and baseline iron status,metabolism in patients withiron deficiency anemia.MethodsA total of 50 patients with iron deficiency anemia and infection admitted to JiangningHospital Affiliated to Nanjing Medical University,Nanjing were selected as infection group between May 2017 and June 2020.Meanwhile,54 patients with irondeficiency anemia were selected as non-infection group.The relevant data between the twogroups were compared.The relationship between infection and iron metabolism indexes was explored.ResultsThe levels ofserum iron,ferritin and transferrin saturation in infection group were lower than those in non-infection group,while the levelof total iron binding capacity was higher than that in non-infection group(t=3.098,15.748,2.449,3.573,all P<0.05).Therewere statistically significant differences in white blood cell count and neutrophil count between the two groups(χ^(2)=4.517,7.429,all P<0.05).Multivariate logistic regression analysis showed that serum iron,ferritin,transferrin saturation andneutrophil count were independent risk factors of infection(P=0.001,<0.001,<0.001,<0.001).Correlation analysis showedthat serum iron,ferritin,and transferrin saturation were negatively correlated with neutrophil count(r=-0.534,-0.609,-0.650,all P<0.05).ConclusionsThe occurrence of infection was closely related to iron metabolism indexes in patients withirondeficiency anemia.Clinically,therapeutic regimens could be formulated based on the above risk factors to prevent infection.
