摘要
目的:探讨红细胞体积(Mcv)、血红蛋白(Hb)、血清β-人绒毛膜促性腺激素(β-HCG)与妊娠期糖尿病患者妊娠结局的关系。方法:选取2021年4月—2023年5月某院收治的妊娠期糖尿病患者96例为观察组,同期健康产检产妇90例为对照组。观察组根据妊娠结局分为不良妊娠结局组(36例)和正常妊娠结局组(60例)。比较2组及不同妊娠结局患者Mcv、Hb及血清β-HCG水平;线性关系检验分析Mcv、Hb及血清β-HCG水平与妊娠期糖尿病患者妊娠结局的关联性;采用多因素Logistic回归方程分析影响妊娠期糖尿病不良妊娠结局的因素;绘制ROC曲线评估Mcv、Hb及血清β-HCG水平对妊娠期糖尿病不良妊娠结局的预测价值。结果:观察组Mcv[(96.41±3.81)fL]、Hb[(130.59±7.69)g/L]水平高于对照组[(87.62±3.68)fL、(116.26±7.88)g/L],β-HCG[(3.34±0.45)mU/L]水平低于对照组[(4.22±0.57)mU/L];不良妊娠结局组Mcv[(93.62±5.57)fL]、Hb[(130.45±7.37)g/L]水平高于正常妊娠结局组[(88.40±5.65)fL、(123.56±7.55)g/L],β-HCG[(3.39±0.65)mU/L]水平低于正常妊娠结局组[(3.68±0.58)mU/L],差异均有统计学意义(P<0.05)。线性关系检验显示,Mcv、Hb及血清β-HCG水平与妊娠期糖尿病不良妊娠结局相关(R^(2)=0.226、0.208、0.134,P均<0.05)。多因素回归分析显示,Mcv、Hb为影响妊娠期糖尿病不良妊娠结局的独立危险因素,β-HCG为保护因素。Mcv、Hb及血清β-HCG联合预测妊娠期糖尿病不良妊娠结局的AUC为0.829,敏感度为75.00%,特异度为78.33%。结论:Mcv、Hb及血清β-HCG水平与妊娠期糖尿病患者不良妊娠结局相关,三者联合检测对妊娠期糖尿病患者不良妊娠结局的预测价值较高。
Objective:To explore the relationship between mean corpuscular volume(MCV),hemoglobin(Hb),serumβ-human chorionic gonadotropin(β-HCG)and the pregnancy outcomes in patients with gestational diabetes mellitus.Methods:96 pregnant women with diabetes admitted to a hospital from April 2021 to May 2023 were selected as the observation group,and 90 healthy pregnant women in the same period were chosen as the control group.The 96 cases in the observation group were re-divided into an adverse pregnancy outcome group(36 cases)and a normal pregnancy outcome group(60 cases).Levels of MCV,Hb,and serumβ-HCG in the two groups of patients with different pregnancy outcomes were compared.Correlations between the levels of MCV,Hb,serumβ-HCG and the pregnancy outcome of patients with gestational diabetes were analyzed with Spearman.Multivariate Logistic regression equation was used to analyze the factors affecting the adverse pregnancy outcome of diabetes in pregnancy.ROC curve was plotted to evaluate the predictive value of MCV,Hb and serumβ-HCG levels in the adverse pregnancy outcome of gestational diabetes.Results:The observation group had higher levels of MCV[(96.41±3.81)fL]and Hb[(130.59±7.69)g/L]than those of the control group[(87.62±3.68)fL and Hb(116.26±7.88)g/L].Theβ-HCG level[(3.34±0.45)mU/L]was lower than that of the control group[(4.22±0.57)mU/L].The adverse pregnancy outcome group had higher levels of MCV[(93.62±5.57)fL]and Hb[(130.45±7.37)g/L]than those of the normal pregnancy outcome group[(88.40±5.65)fL]and[(123.56±7.55)g/L],while theβ-HCG level[(3.39±0.65)mU/L]was lower than that of the normal pregnancy outcome group[(3.68±0.58)mU/L],the differences being statistically significant(P<0.05).Spearman detection showed that levels of Mcv,Hb and BCG were correlated with adwerse pregnancy outcome of gestational diabetes(R^(2)=0.226,0.208,0.134,all P<0.05).Multivariate regression analysis showed that MCV and Hb were independent risk factors for adverse pregnancy outcome of gestational diabetes,andβ-HCG was a protective factor.The AUC of the combined detection of MCV,Hb and serumβ-HCG to predict the adverse pregnancy outcome of gestational diabetes was 0.829,the sensitivity was 75.00%,and the specificity was 78.33%.Conclusion:The levels of MCV and Hb are positively correlated with the adverse pregnancy outcomes in patients with gestational diabetes mellitus,and the level ofβ-HCG is negatively correlated with the adverse pregnancy outcomes.The combined detection of the above indicators has a high predictive value for adverse pregnancy outcomes in diabetes patients in pregnancy.
