心力衰竭并发肺部感染患者外周血PD-1和sCD14与Smac水平变化及意义 被引量:2

Changes and significance of peripheral blood PD-1,sCD14 and Smac in patients with heart failure and lung infection

摘要 目的分析心力衰竭(HF)并发肺部感染患者外周血程序性死亡受体1(PD-1)、可溶性白细胞分化抗原14(sCD14)、线粒体促凋亡蛋白(Smac)水平变化及意义。方法选取2019年6月-2021年11月成都市第七人民医院收治的HF患者为研究对象,根据是否并发肺部感染分为HF合并感染组(n=73)和HF组(n=185),另选取同期本院行健康体检人群70名作为对照组。检测三组外周血PD-1、sCD14、Smac水平,并采用Pearson相关性分析3项指标间的关系。比较不同肺部感染严重程度HF患者外周血PD-1、sCD14、Smac水平,采用Spearman相关分析外周血PD-1、sCD14、Smac水平与肺部感染严重程度的关系。结果与对照组比较,HF组、HF合并感染组患者PD-1、sCD14水平升高,Smac水平降低,差异有统计学意义(F=396.776、93.895、359.859,P<0.05)。PD-1、sCD14水平均与Smac水平成负相关(r=-0.290、-0.247,P<0.05)。HF合并肺部感染患者的PD-1、sCD14水平与肺部感染程度成正相关(r=0.354、0.576,P<0.05),Smac与肺部感染程度成负相关(r=-0.557,P<0.05)。不同肺部感染病情的HF患者PD-1、sCD14、Smac水平比较差异均有统计学意义(χ^(2)=9.744、14.713、14.209,P均<0.05)。结论HF并发肺部感染患者外周血PD-1、sCD14水平显著升高,Smac水平显著降低,均与HF患者肺部感染发生、发展有关。 Objective To analyze the changes and significance of peripheral blood programmed cell death protein 1(PD-1),soluble leukocyte differentiation antigen 14(sCD14)and second mitochondria-derived activator of caspases(Smac)in patients with heart failure(HF)and lung infection.Methods Patients with HF admitted to the hospital were enrolled as the research objects between June 2019 and November 2021.According to presence or absence of lung infection,they were divided into HF-infection group(n=73)and HF group(n=185).A total of 70 healthy controls during the same period were enrolled as control group.The levels of peripheral blood PD-1,sCD14 and Smac in the three groups were detected.And their relationships were analyzed by Pearson correlation analysis.The levels of peripheral blood PD-1,sCD14 and Smac in HF patients with different severity of lung infection were compared.The relationships between PD-1,sCD14,Smac and the severity of lung infection were analyzed by Spearman correlation analysis.Results Compared with control group,levels of PD-1 and sCD14 were significantly increased,while Smac level was significantly decreased in HF group and HF-infection group(F=396.776,93.895,359.859,P<0.05).The levels of PD-1 and sCD14 were negatively correlated with Smac level(r=-0.290,-0.247,P<0.05).The levels of PD-1 and sCD14 were positively correlated with the severity of lung infection(r=0.354,0.576,P<0.05),while Smac level was negatively correlated with lung infection(r=-0.557,P<0.05).There were significant differences in PD-1,sCD14 and Smac levels among HF patients with different severity of lung infection(χ^(2)=9.744,14.713,14.209,all P<0.05).Conclusion The levels of peripheral blood PD-1 and sCD14 significantly increased,while Smac level significantly decreased in patients with HF and lung infection,which were all related to the occurrence and development of lung infection in HF patients.
