摘要
目的分析广州地区学龄儿童(6-12岁)鼻腔中耐甲氧西林金黄色葡萄球菌(MRSA)的携带情况,并进行药敏实验及多位点序列基因分型(MLST)。方法采集学龄儿童鼻拭子1029份,接种于哥伦比亚血琼脂,挑取具有金黄色葡萄球菌菌落形态特征的细菌进行全自动微生物鉴定系统(VITEK)鉴定,对已鉴定为金黄色葡萄球菌的菌株进行药敏实验并对MRSA菌株进行耐药分析及MLST基因分型。结果学龄儿童鼻腔携带金黄色葡萄球菌率为35.3%(363/1029),MRSA携带率为4.5%(46/1029),MRSA菌株对青霉素和头孢西丁耐药率为100.0%,对克林霉素和红霉素耐药率分别为69.6%、63.0%,对四环素耐药率为10.9%,对磺胺甲恶唑-甲氧苄啶、庆大霉素耐药率为2.2%,对利福平、左旋沙星、莫西沙星、西普沙星耐药率为4.4%,未检出万古霉素、呋喃妥因、替加环素、利奈唑胺和奎奴普丁/达福普汀耐药的菌株。46株MRSA菌株鉴定为12种ST基因型,主要的基因型为ST45型占58.7%,ST59型占15.2%,ST338型占4.4%,ST508型占4.4%。其中ST45、ST59基因型均对四环素耐药(P=0.0009、0.0346),其他抗生素的耐药各基因型差异无统计学意义(P>0.05)。结论广州健康学龄儿童鼻腔中MRSA携带率为4.5%,其主要基因型为ST45型、ST59型。应加强学龄儿童健康宣教,关注手鼻卫生,减少MRSA在儿童中的交叉感染。
Objective To investigate the prevalence of methicillin-resistant Staphylococcus aureus(MRSA)in nasal cavity of school-age children in Guangzhou area,and to conduct drug sensitivity test and multilocus sequence genotyping.Methods A total of 1029 nasal swabs from school-age children were collected and inoculated on Colombian blood plates.The isolates were identified by VITEK mass spectrometry,and methicillin-resistant Staphylococcus aureus antibiotic resistance was analyzed and MLST typing was performed.Results The rate of Staphylococcus aureus in nasal cavity was 35.3%(363/1029),the rate of MRSA was 4.5%(46/1029),the drug resistance rate of MRSA strain to penicillin and cefoxitin was 100.0%,the drug resistance rate to clindamycin and erythromycin was 69.6%and 63.0%,respectively.The drug resistance rate to tetracycline was 10.9%,the drug resistance rate to sulfamethoxazole-trimethoprim and gentamicin was 2.2%,and the drug resistance rate to rifampicin,levofloxacin,moxifloxacin and siprofloxacin was 4.4%.No resistant strains of vancomycin,furantoin,tigacycline,linezolid and quinupuptin/daphoputin were detected in this study.The 46 strains of MRSA were divided into 12 ST genotypes,and the main genotypes were ST45(58.7%),ST59(15.2%),ST338(4.4%)and ST508(4.4%).Among antibiotic resistance,the resistance of ST45 and ST59 to tetracycline was different from other genotypes(P=0.0009,0.0346),and there was no statistical difference in drug resistance among other antibiotics.Conclusions The carrying rate of MRSA in nasal cavity of healthy school-age children in Guangzhou was 4.5%,and the main genotypes were ST45 and ST59.Health education for school-age children should be strengthened,and attention should be paid to hand and nose hygiene to reduce the cross infection of MRSA in children.
