摘要
目的对比分析肠道病毒71型(EV71)与柯萨奇病毒A组6型(CA6)/柯萨奇病毒A组10型(CA10)/柯萨奇病毒A组16型(CA16)重叠感染及EV71单独感染的手足口病(HFMD)患者的临床表现及实验室检查等。方法收集成都市公共卫生临床医疗中心2019年1月-2021年9月收治的病毒检测确诊病例1510例,分析2019年全年患者区域、时间及病毒型特点,并选择EV71重叠感染患者(重叠感染组)与EV71单独感染者(普通组)进行临床特征对比。结果2019年手足口病在5-7月和10-11月出现2个高发病期,其中CA6型为成都市全年发生率最高的病毒型,CA6和CA10基本全年都有发现。重叠感染组和普通组比较,口腔、臀部疱疹、发热、扁桃体发炎、心肌炎的发生率差异均有统计学意义(χ^(2)=0.721、0.667、1.227、1.432、6.408,P均<0.05)。重叠感染组白细胞计数、C反应蛋白、乳酸脱氢酶超正常值例数占比较普通组明显升高,差异均有统计学意义(χ^(2)=1.191、1.388、1.005,P均<0.05)。结论EV71重叠感染患者较EV71单独感染患者皮疹和伴随其他疾病的风险更高,对于重叠感染患者应该加强监测,早期治疗,预防患者病情转恶。
Objective To compare the clinical characteristics and laboratory examination between enterovirus(EV71)co-infected with coxsackievirus A6(CA6)/CA10/CA16 hand-foot-mouth disease(HFMD)patients and EV71 single infection patients.Methods From January 2019 to September 2021,1510 cases were confirmed by virus detection in the hospital of Public Health Clinical Center of Chengdu.The regional,time and viral type characteristics of patients in 2019 were analyzed emphatically.The clinical characteristics of patients with EV71 superinfection and EV71 single infection were compared.Results There were two high incidence periods of HFMD in our city from May to July and from October to November.CA6 was the highest incidence in the whole year in Chengdu city,and CA6 and CA10 were found in the whole year.The occurrence rate of oral cavity herpes,buttock herpes,fever,tonsillitis and myocarditis in EV71 co-infection group was significantly higher than that in EV71 single infection group(χ^(2)=0.721,0.667,1.227,1.432,6.408,all P<0.05);The ratio of number of abnormal values of white blood cells count,C-reactive protein,and lactate dehydrogenase in EV71 co-infection group was significantly higher than those in the single infection group(χ^(2)=1.191,1.388,1.005,all P<0.05).Conclusions The EV71 co-infection patients had a higher risk of skin herpes and concomitant infection with other diseases than the single infection EV71 patients.Patients with EV71 co-infection should be carefully monitored and treated early in order to prevent the patients from getting worse.
