摘要
介绍郑宝林教授分期论治痛风的临床经验。郑宝林基于整体观念,认为痛风发病具有以“湿浊痰瘀为标,脾肾亏虚为本”的特点,遵循湿热壅盛-痰浊阻滞-肾虚瘀毒的传变规律,故提出分期论治的总体治则,湿热壅盛期以四妙散加减清热解毒,利湿通络;痰浊阻滞期以陈夏六君汤加减健脾化痰,化瘀通络;肾虚瘀毒期以补肾祛瘀汤加减扶正祛邪,标本兼治。临证擅用藤类药、消石药,关节疼痛甚者,分期用三藤,即青风藤、络石藤、鸡血藤;痛风石沉积甚者,善用金钱草、石韦、鸡内金。
This paper introduces the clinical experience of Professor ZHENG Baolin treating gout by stages.Based on the holistic concept,ZHENG Baolin believes that the incidence of gout is characterized by"dampness,phlegm and stasis being the symptom and spleen-kidney deficiency being the root cause",and follows the law of transmission and change of damp-heat accumulation-phlegm-turbidity obstructionkidney deficiency and stasis toxin.Therefore,he proposed the overall principle of treating diseases by stages.During the period of damp-heat accumulation,Modified Simiao Powder can be used to clear heat and resolve toxins and drain dampness and unblock the collaterals;during the period of phlegm-turbidity obstruction,Modified Chenxia Liujun Decoction can be used to fortify the spleen and dissolve phlegm and dissolve stasis and unblock the collaterals;during the period of kidney deficiency and stasis toxin,Modified Bushen Quyu Decoction can be used to reinforce healthy qi and dispel pathogen,treating the root cause and symptom simultaneously.He is good at treating clinical syndromes by rattan medicine and medicines for eliminating gout stone.For patients with serious joint pain,Sinomenii Caulis,Trachelospermi Caulis et Folium,and Spatholobi Caulis are given by stages;for patients with serious gout stone deposition,the patients should be given Lysimachiae Herba,Pyrrosiae Folium,and Galli Gigeriae Endothelium Corneum.
