摘要
目的:观察通督调神针法联合康复训练治疗缺血性脑卒中患者吞咽障碍的临床疗效。方法:选择2017年8月至2019年8月我院收治的缺血性脑卒中患者80例,根据治疗方式不同分为对照组(40例)和观察组(40例),对照组给予常规康复训练,观察组采用通督调神针法联合康复训练,连续治疗4周。比较两组患者治疗后的临床疗效及治疗前后洼田饮水试验评分、标准吞咽功能评价量表(SSA)评分、电视透视吞咽功能检查(VFSS)评分和血清脑源性神经营养因子(BDNF)水平变化情况。结果:两组患者治疗后,洼田饮水试验评分、SSA评分均较治疗前有明显下降,且观察组的洼田饮水试验评分、SSA评分比对照组更低,差异有统计学意义(P<0.05);两组VFSS评分及血清BDNF水平较治疗前升高,且观察组升高趋势更显著,差异有统计学意义(P<0.05);治疗后与对照组比较,观察组治疗有效率(92.50%)高于对照组(72.50%),差异有统计学意义(P<0.05)。结论:通督调神针法联合康复训练治疗缺血性脑卒中患者吞咽障碍临床效果显著,有效改善症状,提高吞咽功能,临床应用价值显著。
Objective:To study the efficacy of applying Tongdu Tiaoshen Acupuncture Method combined with rehabilitation training in the treatment of ischemic stroke patients with dysphagia.Methods:A total of 80 ischemic stroke patients with dysphagia in traditional Chinese medicine department between August 2017 and August 2019 were enrolled in the present study,and divided into a controlled group(n=40)and an observation group(n=40)by using stratified randomization methods.The patients of the controlled group received rehabilitation training.The patients of the observation group were treated by Tongdu Tiaoshen Acupuncture Method on the basic treatment as those mentioned in the controlled group.All patients were treated for 4 weeks.The swallowing ability was tested by water swallowing test,standardized swallowing assessment(SSA),video fluoroscopic swallowing study(VFSS).And serum brain derived neurotrophic factor(BDNF)level was assayed by enzyme linked immunosorbent assay.The effect of swallowing ability was compared between the two group after treatment.Results:After 4 weeks’treatment,the scores of water swallowing test,SSA in the two groups were decreased considerably(P<0.05),and VFSS scores as well as serum BDNF were obviously increased(P<0.05)compared with their own pre-treatment.The therapeutic effect of the observation group(92.50%)was significantly superior to that of the controlled group(72.50%)in the effective rate(P<0.05),and in lowering the scores of water swallowing test and SSA(P<0.05),in up-regulating the scores of VFSS and serum BDNF level(P<0.05).Conclusions:The efficacy of applying Tongdu Tiaoshen Acupuncture Method combined with rehabilitation training in the treatment of ischemic stroke with dysphagia is significantly.It is effective to improve symptoms,promote the improvement of swallowing ability.The clinical application value is significantly.
