摘要
目的:探究膀胱癌术后应欲消癥肾气汤结合中医埋针治疗效果。方法:纳入98例膀胱癌患者(2019年1月~2020年4月收治),将患者以随机数字表法分为对照组与观察组,各49例;对照组患者采取经尿道电切术+丝裂霉素灌注治疗,观察组经尿道电切术+丝裂霉素灌注并联合消癥肾气汤及中医埋针治疗,数据比较:疗效及并发症、治疗前后中医证候积分(尿频尿急尿痛、排尿困难、腰酸、肢体逆冷等)及血清肿瘤坏死因子α(TNF-α)水平变化、免疫功能指标变化及生活质量(QLQ-BLM30)评分变化,随访1年,比较患者1年癌症复发率。结果:与对照组患者相比(63.27%)、(16.33%),观察组有效率(81.63%)更高,并发症率(2.04%%)更低(P<0.05);治疗前,两组患者中医证候积分(尿频尿急尿痛、排尿困难、腰酸、肢体逆冷等)、TNF-α水平、CD4^(+)、CD8^(+)、QLQ-BLM30评分等指标比较无显著性差异(P>0.05);治疗后各组患者中医证候积分(尿频尿急尿痛、排尿困难、腰酸、肢体逆冷等)、TNF-α水平、CD4^(+)、CD8^(+)、QLQ-BLM30评分等指标均改善,观察组患者治疗后中医证候积分(尿频尿急尿痛、排尿困难、腰酸、肢体逆冷等)、TNF-α水平、CD4^(+)、CD8^(+)、QLQ-BLM30评分等指标优于对照组(P<0.05);1年后随访显示观察组患者复发率低于对照组(P<0.05)。结论:膀胱癌术后应用消癥肾气汤结合中医埋针治疗效果佳,患者免疫功能及生活质量提升,1年复发率降低,安全可靠。
Objective:To explore the efficacy of using Xiaozheng Shenqi Decoction combined with traditional Chinese medicine needle embedding in the treatment of bladder cancer.Methods:A total of 98 patients with bladder cancer(admitted from January 2019 to April 2020)were enrolled and divided into a controlled group and an observation group by the random number table method,with 49 cases in each group.The controlled group was treated with transurethral resection+mitomycin infusion,while the observation group was treated with transurethral resection+mitomycin infusion combined with Xiaozheng Shenqi Decoction and traditional Chinese medicine needle embedding.Data comparison:Efficacy and complications,TCM syndrome scores(frequency of urination,pain of urination,dysuria,lumbar acid,limb retrocoldness,etc.),changes in serum tumor necrosis factorα(TNF-α)level,changes in immune function indexes and quality of life(QLQ-BLM30)score before and after treatment.Patients were followed up for 1 year,and cancer recurrence rate of 1 year was compared.Results:Compared with the controlled group(63.27%)and(16.33%),the effective rate of the observation group(81.63%)was higher,and the complication rate(2.04%)was lower,P<0.05.Before treatment,the scores of TCM syndrome(frequency of urination,pain of urination,dysuria,lumbar acid,limb retrocoldness,etc.),TNF-αlevel,CD4^(+),CD8^(+),QLQ-BLM30 score and other indicators in the 2 groups were compared,P>0.05.After treatment,the TCM syndrome score(frequency of urination,pain of urination,dysuria,lumbar acid,limb retrocoldness,etc.),TNF-αlevel,CD4^(+),CD8^(+),QLQ-BLM30 score and other indicators were improved in all groups.The observation group was better than the controlled group after treatment in TCM syndrome score(frequency of urination,pain of urination,dysuria,lumbar acid,limb cold,etc.),TNF-αlevel,CD4^(+),CD8^(+),QLQ-BLM30 score,P<0.05.One year later,the follow-up showed that the recurrence rate of the observation group(0.00%)was lower than that of the controlled group(8.16%),P<0.05.Conclusion:Postoperative treatment of bladder cancer with Xiaozheng Shenqi Decoction combined with traditional Chinese medicine needle embedding has good effect,can improve the immune function and quality of life of patients,and can reduce the one-year recurrence rate,which is safe and reliable.
