摘要
膝骨关节炎(KOA)属“痹证”“痛痹”等范畴,其病因为气虚血虚、脏腑失调、肝肾亏虚、脾胃亏虚,病机为经络气血淤结,痰瘀互结,治疗原则应以通痹除湿、活血散结、补益肝肾为主。独活寄生汤为《备急千金要方》中记载的经典名方,具有祛风湿、止痹痛及补气血等功效,在临床被广泛应用于风湿性关节炎(RA)、KOA及腰肌劳损等疾病治疗,可有效改善患者疼痛症状,促使功能恢复正常。本文将从中医认知、作用机制、治疗现状等方面对独活寄生汤治疗KOA的研究进展作一述评,以期为临床合理用药提供循证依据。
Knee osteoarthritis(KOA)belongs to the categories of arthralgia syndrome and pain paralysis.The etiology is qi-blood deficiency,viscera disorders,liver-kidney deficiency and spleen-stomach deficiency.The pathogenesis is qi-blood stasis in meridians and stasis-phlegm.The main treatment principles include relieving paralysis and dampness,activating blood and removing stasis,and reinforcing liver and kidney.Duhuo Jisheng Decoction is a classic prescription recorded in Valuable Prescriptions for Emergency,which has the functions of dispelling rheumatism,relieving pain paralysis and invigorating qi-blood.In clinical treatment,it is widely used in rheumatoid arthritis(RA),KOA and lumbar muscle strain,which can effectively improve pain symptoms and promote function recovery in patients.This article will review the research progress about Duhuo Jisheng Decoction in the treatment of KOA from the aspects of TCM cognition,mechanism of action and current treatment status in order to provide evidence-based basis for clinical rational medication.
