摘要
目的小儿过敏性疾病与后天环境及先天遗传等均存在一定关联,对儿童身心健康及生活质量造成严重影响。本研究探讨血清特异性免疫球蛋白E(IgE)与免疫球蛋白G(IgG)联合检测对小儿过敏性疾病的诊断价值。方法选择2015-10-17-2018-11-25本院收治的37例吸入性过敏患儿(A组),50例食物性过敏患儿(B组)和同期本院收治的40例非过敏性疾病患儿(对照组)为研究对象;入选者均接受血清特异性IgE和IgG检测,分析IgE联合IgG检测在小儿过敏性疾病诊断价值。结果A组、B组和对照组血清IgE水平分别为(4.22±1.03)、(6.53±1.10)和(2.60±0.85)g/L,F=173.203,P<0.001;IgG水平分别为(205.63±45.21)、(321.02±40.58)和(145.23±30.13)IU/mL,F=236.367,P<0.001。血清IgE和IgG单独及联合诊断小儿过敏性疾病的曲线下面积分别为0.806、0.789和0.862,均有一定预测价值;联合诊断灵敏度为94.25%,高于血清IgE(82.76%)和IgG(85.06%)单独诊断,χ2=5.649,P<0.05。与金标准诊断结果比较,联合试验的一致性检验Kappa值为0.792,优于血清IgE(0.579)和IgG(0.669)。结论食物过敏原与吸入性过敏原的过敏性疾病患儿血清IgE、IgG水平异常升高,联合测定两项指标对过敏性疾病的诊断具有重要价值。
Objective Pediatric allergic diseases are related to the acquired environment and congenital genetics,which have a serious impact on children’s physical and mental health and quality of life.This study aimed to explore the diagnostic value of combined detection of serum specific immunoglobulin E(IgE)and immunoglobulin G(IgG)in children with allergic diseases.Methods A total of 37 children with inhalation allergy(A group),50 children with food allergy(B group)and40 children admitted to our hospital from October 17,2015 to November 25,2018 were selected as the subjects of the study.All selected candidates needed to be tested for serum specific IgE and IgG to analyze the diagnostic value of serum IgE combined with IgG testing in children with allergic diseases.Results Serum IgE levels in group A,group B and control group were(4.22±1.03),(6.53±1.10)and(2.60±0.85)g/L,F=173.203,P<0.001.IgG levels were(205.63±45.21),(321.02±40.58)and(145.23±30.13)IU/ml,F=236.367,P<0.001.The area under the curve for the diagnosis of pediatric allergic diseases by serum IgE and IgG alone and in combination were 0.806,0.789 and 0.862,which had certain predictive value.The combined diagnosis sensitivity was 94.25%,which was higher than that of serum IgE(82.76%)and IgG(85.06%)alone,χ2=5.649,P<0.05.Compared with the gold standard diagnosis result,the consistency test Kappavalue of the combined test was 0.792,which was better than serum IgE(0.579)and IgG(0.669).Conclusions The levels of serum IgE and IgG in children with allergic diseases of food allergens and inhaled allergens are abnormally increased.The combined determination of these two indicators is of great value for the diagnosis of allergic diseases.
