Effect of different blood purification methods on trace elements in patients with liver failure

摘要 目的:探讨不同血液净化方式对肝功能衰竭患者微量元素的影响。方法:选取2017-04—2018-05收治的肝功能衰竭患者108例作为研究对象,按就诊顺序单双号分为对照组(54例)和观察组(54例)。对照组患者在内科综合治疗的基础上,予以血浆置换治疗;观察组患者在内科综合治疗的基础上,予以双重血浆分子吸附术治疗。对比患者治疗前后的丙氨酸氨基转移酶(ALT)、凝血酶原时间(PT)、血氨(BA)、铁、镁、锌、铜、硒、白细胞介素-10(IL-10)。结果:2组患者治疗后的ALT、PT、BA、IL-10水平均显著低于治疗前,同时,治疗后观察组患者的ALT、PT、BA、IL-10水平均显著低于对照组;观察组治疗后的铁、镁、锌、硒等微量元素水平显著高于治疗前,且铜元素水平显著低于治疗前;同时,治疗后观察组的铁、镁、锌、硒等微量元素水平显著高于对照组,且铜元素水平显著低于对照组;差异有统计学意义(P<0.05)。结论:相对于血浆置换术,双重血浆分子吸附术能够有效改善患者肝功能,有助于患者体内铜元素的排出,同时有利于镁、铁、锌、硒等微量元素的吸收、储存,从而有效改善患者微量元素的平衡,值得推广应用。 Objective:To explore the effect of different blood purification methods on trace elements in liver failure patients.Method:A total of 108 patients with liver failure admitted to our hospital from April 2017 to May 2018 were selected as the study subjects.They were divided into control group(n=54)and observation group(n=54)according to the order of visiting.Patients in the control group were treated with plasma exchange on the basis of comprehensive medical treatment,while patients in the observation group were treated with double plasma molecular adsorption on the basis of comprehensive medical treatment.The levels of ALT,PT,BA,iron,magnesium,zinc,copper,selenium and IL-10 were compared before and after treatment.Result:The levels of ALT,PT,BA and IL-10 in the two groups after treatment were significantly lower than those before treatment.At the same time,the levels of ALT,PT,BA and IL-10 in the observation group were significantly lower than those in the control group after treatment.The levels of trace elements such as iron,magnesium,zinc and selenium in the observation group after treatment were significantly higher than those before treatment,and the level of copper was significantly lower than that before treatment.At the same time,the levels of trace elements such as iron,magnesium,zinc and selenium in the observation group after treatment were significantly higher than those in the control group,and the level of copper was significantly lower than that in the control group;the difference was statistically significant(P<0.05).Conclusion:Compared with plasma replacement,double plasma molecular adsorption can effectively improve the liver function of patients with liver failure,contribute to the excretion of copper elements in patients,and also facilitate the absorption and storage of magnesium,iron,zinc,selenium and other trace elements,thus effectively improving the balance of trace elements in patients,which might be worthy of promotion and application.
