摘要
目的探讨固本平喘膏治疗肺胀肺肾气虚证的临床疗效。方法选取2017年1月—2019年1月湖南中医药高等专科学校附属第一医院收治的肺胀患者130例为研究对象,采用随机数字表法分为对照组和研究组,每组65例。对照组予沙美特罗替卡松粉吸入剂50/500μg吸入,研究组在对照组基础上应用固本平喘膏进行治疗,持续用药60 d。对比两组患者治疗前后肺功能指标〔第1秒用力呼气末容积(FEV1)、第1秒用力呼气末容积占预计值百分比(FEV1%)〕变化情况及6 min步行距离(6MWT),治疗前后应用CAT评分衡量患者的生活质量,记录1年内急性加重次数。结果治疗前,两组FEV1、FEV1%、6MWT、CAT评分比较,差异无统计学意义(P>0.05);治疗后,研究组FEV1、FEV1%、6MWT高于对照组,CAT评分低于对照组(P<0.05)。两组治疗后FEV1、FEV1%、6MWT高于治疗前,CAT评分低于治疗前(P<0.05)。研究组1年内急性加重例数少于对照组(P<0.05)。结论应用固本平喘膏治疗肺胀肺肾气虚证取得了较为满意的效果,不仅可以达到补肺益肾的效果,提高了患者的自身免疫机制,而且降低了药材的成本,提高了患者的生活质量,减少肺胀患者急性加重次数,应该在临床试验中推广应用。
Objective To investigate the clinical effect of Guben Pingchuan ointment in the treatment of lung emphysema,lung and kidney Qi deficiency.Methods 130 patients with lung emphysema,lung and kidney Qi deficiency in the First Affiliated Hospital of Hunan Traditional Chinese Medicine College from January 2017 to September 2019 were selected,and they were divided into control group and research group according to the random number table method,65 cases in each group.The patients in the control group were treated with salmeterol xinafoate and fluticasone propionate powder inhalation 50/500μg,while the patients in the research group were treated with Guben Pingchuan ointment on the basis of the control group.Continuous medication for 60 days.The lung function indexes(FEV1,FEV1%),6MWT,were compared between the two groups before and after treatment.CAT score was used to measure the quality of life of patients before and after treatment,the number of acute exacerbations in 1 year were record.Results There was no statistically significant difference in the FEV1,FEV1%,6MWT and CAT score between the two groups before treatment(P>0.05);after treatment,the FEV1,FEV1%,6MWT in the research group were higher than those in the control group,and the CAT score in the research group was lower than that in the control group(P<0.05).The number of acute exacerbations in 1 year in the research group was lower than that in the control group(P<0.05).Conclusion Guben Pingchuan ointment has achieved satisfactory results in the treatment of lung emphysema,lung and kidney Qi deficiency.It can not only strengthen the lung and kidney,improve the patient's autoimmune mechanism,but also reduce the cost of medicinal materials and improve the patient's quality of life.Therefore,it should be popularized in clinical trials.
