摘要
目的探讨远红外线照射在动静脉内瘘扣眼穿刺隧道形成中的应用价值。方法选取于2019年3月-2020年5月我院进行动静脉瘘血液透析的100例患者作为研究对象。将患者随机分为观察组和对照组,各50例。对照组患者采用动静脉内瘘扣眼穿刺法,观察组在对照组的基础上给予远红外线照射。比较3个月后两组患者治疗前后的透析效果、血流量改善情况、治疗预后情况以及疼痛和日常生活能力。结果观察组治疗3个月后一次穿刺成功率(92%)高于对照组(78%)(P<0.05),穿刺部位渗血率(2%)及拔针后按压时间〔(7.46±0.09)min〕均低于对照组〔12%、(14.59±0.11)min〕(P<0.05);观察组治疗3个月后头静脉血流量(540.25±53.29)mL/min高于对照组(412±51.12)mL/min(P<0.05),头静脉壁厚度(1.46±0.09)cm低于对照组(1.59±0.11)cm(P<0.05);观察组治疗3个月后并发症发生率(10%)低于对照组(48%)(P<0.05);观察组治疗3个月后1个月VAS评分〔(2.17±0.12)分〕低于对照组〔(7.20±0.66)分〕(P<0.05);观察组治疗3个月后1个月ADL量表评分〔(79.31±12.94)分〕高于对照组〔(63.29±10.43)分〕(P<0.05)。结论将远红外线照射应用在动静脉内瘘扣眼穿刺隧道形成中显著提高透析效果率,缩短了疼痛时间及拔针后按压时间,显著改善血流量,降低术后不良反应,改善患者生活能力,值得推广应用。
Objective This paper aims to explore the application value of far infrared radiation in the formation of arteriovenous fistula buttonhole puncture tunnel.Methods 100 patients with arteriovenous fistula hemodialysis in our hospital from March2019 to may 2020 were selected as the research objects.The patients were randomly divided into observation group and control group,50 cases in each group.The control group was treated with arteriovenous fistula buttonhole puncture,and the observation group was given far infrared radiation on the basis of the control group.After 3 months,the dialysis effect,blood flow improvement,treatment prognosis,pain and activities of daily living were compared between the two groups before and after treatment.Results The success rate of one puncture in the observation group was 92%higher than that in the control group 78%(P<0.05),the bleeding rate at puncture site and the pressing time after needle extraction were(7.46±0.09)min lower than those in the control group〔12%,(14.59±0.11)min〕(P<0.05);the blood flow of cephalic vein in the observation group was(540.25±53.29)mL/min,which was higher than that of the control group(412±51.12)mL/min(P<0.05)The wall thickness of the observation group was(1.46±0.09)cm lower than that of the control group(1.59±0.11)cm,(P<0.05);the complication rate of the observation group was 10%lower than that of the control group 48%(P<0.05);the VAS score of the observation group was(2.17±0.12)lower than that of the control group(7.20±0.66)(P<0.05);the daily living ability of the observation group was(79.31±12.94)points higher than that of the control group after 3 months of treatment(63.29±10.43)(P<0.05).Conclusion The application of far infrared radiation in the formation of arteriovenous fistula buttonhole puncture tunnel can significantly improve the dialysis effect rate,shorten the pain time and pressing time after pulling out the needle,significantly improve the blood flow,reduce the postoperative adverse reactions,and improve the living ability of patients,which is worthy of promotion and Application.
