摘要
目的研究3D打印技术辅助人工膝关节置换术治疗膝关节骨性关节炎与常规治疗方案的临床效果。方法选取2019年12月-2023年2月在我院接受单侧膝关节置换术的90例膝关节骨性关节炎患者,根据手术方式将其分为对照组及研究组。对照组行人工全膝关节置换术,研究组行3D打印技术辅助人工全膝关节置换术,通过倾向性评分匹配筛选两组患者,分析对比两组围术期指标,置换前、置换后2周膝关节活动范围、股骨与下肢力线恢复情况及置换前、置换2月时膝关节功能。结果研究组手术时间短于对照组(P<0.05);研究组出血量少于对照组(P<0.05)。手术前,两组患者膝关节活动范围对比无显著差异(P>0.05);术后2周时,两组患者膝关节活动范围均有提升(P<0.05),两组对比无显著差异(P>0.05)。手术前,两组患者股骨与股骨机械轴夹角对比无显著差异(P>0.05),术后2周时,两组患者股骨与股骨机械轴夹角均有下降(P<0.05),两组对比无显著差异(P>0.05)。手术前,两组患者牛津膝关节评分量表(0xford knee score,OKS)各维度评分及总评分对比无显著差异(P>0.05);术后2月时,两组患者OKS各维度评分及总评分均有下降(P<0.05),对比无显著差异(P>0.05)。结论人工全膝关节置换术与3D打印技术辅助人工全膝关节置换术对比两者均可改善膝关节活动范围、改善下肢力线恢复,提升膝关节功能,两者对比无显著差异,但3D打印技术辅助人工全膝关节置换术具有手术时间短、出血量少的优点,更适合于基础状态较差,需尽快完成手术的患者。
Objective:To study the effect of 3D printing-assisted artificial total knee arthroplasty in patients with knee osteoarthritis.Methods:A total of 90 patients with knee osteoarthritis who received unilateral knee arthroplasty in our hospital from December 2019 to February 2023 were selected,and were divided into a control group and a study group according to the surgical method.The control group underwent artificial total knee arthroplasty,and the study group underwent 3D printing-assisted artificial total knee arthroplasty.The patients in both groups were screened by matching the inclination score,and the perioperative indexes,knee range of motion,femur and lower limb line of force recovery at pre-and 2 weeks post-surgery,and knee function at pre-and 2 months post-surgery were analyzed and compared between the two groups.Results:The operation time of the study group was shorter than that of the control group(P<0.05).The blood loss in the study group was less than that in the control group(P<0.05).Before surgery,there was no significant difference between the two groups(P>0.05);For 2 weeks after surgery,the knee joint motion range between the two groups increased(P<0.05),and there was no significant difference between groups(P>0.05).Before surgery,there was no significant difference in the mechanical shaft angle between the two groups(P>0.05);For 2 weeks after surgery,in the mechanical shaft angle between the two groups decreased(P<0.05),and there was no significant difference between the two groups(P>0.05).Before surgery,there were no significant differences in the scores in all dimensions and the total scores of 0xford knee score(OKS)between the two groups(P>0.05).For 2 months after surgery,the scores in all dimensions and the total score of OKS between the two groups decreased(P<0.05),but there was no significant difference in comparison(P>0.05).Conclusion:The comparison between artificial total knee arthroplasty and 3D printing-assisted total knee arthroplasty can improve the range of knee joint motion,improve the recovery of lower limb force line,and enhance the knee joint function,with no significant difference between the two groups.However,3D printing-assisted total knee arthroplasty has the advantages of short operation time and less blood loss,which is more suitable for patients with poor basic state who need to complete surgery as soon as possible.
