摘要
目的探讨急性阑尾周围脓肿Ⅰ期手术治疗的临床效果。方法对119例急性阑尾周围脓肿Ⅰ期手术治疗的病例资料进行回顾性分析。结果全组病例均获一期治愈,术中无1例发生医源性及/或副损伤。切口甲级愈合96例(约占手术例数的80%),乙级愈合15例(约占手术例数的13%),丙级愈合8例(约占手术例数的7%);术后并发粘连性及早期炎性肠梗阻9例(约占手术例数的8%),均经保守治愈;无腹腔感染扩散、内出血及肠瘘等并发症;住院时间7-16天,平均9天。结论急性阑尾周围脓肿Ⅰ期手术治疗临床效果满意,只要术中严格遵守阑尾脓肿的手术原则,谨慎认真操作,一般不易发生严重并发症,却使患者受益颇多,尤其在就医条件差的偏远贫困地区更宜放宽Ⅰ期手术指征。
Objective To exlpore the clinical effects of the one-stage surgical treatment for acute periappendicural absecess.Methods Data of 119 cases with acute periappendicural absecess and being treated with one-stage surgical treatment were retrospectively analyzed. Results All cases were cured in the first stage, and no iatrogenic injury and side injury. There were 96 cases with wound healing in Class-A(accounting for about 80% of the surgical cases), 15 cases in Class-B wound healing(accounting for about 13% of the surgical cases), and 8 cases in Class-C wound healing(accounting for about 7% of the surgical cases). There were 9 cases having postoperative adhesion and early inflammatory ileus(about 8% of the cases), all of which were cured conservatively. And there were no abdominal infection, internal bleeding, intestinal fistula and other complications;Length of stay ranged from 7 to 16 days, and average day was 9 days. Conclusions The clinical effect on the one-stage surgical treatment for the acute periappendicural absecess is atisfactory. as long as patients can obey the principle of appendix abscess surgery strictly and operation were carried out cautiously. Serious complications rarely occurred, and it benefited patients a lot. So we should relax the one-stage surgical indications, especially in the remote poor areas with poor medical condition.
