摘要
目的:本研究旨在调查伴有抑郁症状的医学生中精神心理问题预期污名的严重程度。方法:在我国三所医学院校中招募自愿接受调查的医学生,采用患者健康问卷(Patient Health Questionnaire,PHQ-9)对医学生的抑郁症状进行筛查评估,采用预期污名相关条目对医学生的预期精神心理问题污名情况进行测量,选取其中1720名有抑郁症状(PHQ-9≥7)的医学生纳入本研究,采用频数频率的方式对伴抑郁症状医学生的人口学特征和预期污名情况进行描述,应用独立样本t检验、单因素方差分析比较不同人群特征的预期污名严重程度,运用多元线性回归分析探索PHQ-9评分与预期污名的关系。结果:有抑郁症状的医学生中存在着一定程度的预期污名,且他们最担心来自上级老师的、和规培与研究生申请中的偏见和歧视。男性、汉族、精神医学专业、有过临床实习经历、家庭所在地为城市的伴抑郁症状医学生的预期污名程度高于相应组别的学生(P<0.05)。多元线性回归分析显示,PHQ-9得分与预期污名调查问卷得分呈显著正相关关系(P<0.001)。结论:抑郁筛查为阳性的医学生中存在对周围环境中污名的担心,未来研究可进一步针对医学院中不同人群(如学生、教师、辅导员等)进行污名情况的调查与干预措施的制定。
Objective:To investigate the anticipated stigma among medical students with depressive symptoms.Methods:Medical students were recruited from three medical universities in China.We used the Patient Health Questionnaire(PHQ-9)and anticipated stigma questionnaire to explore depressive symptoms and anticipated stigma of medical students.In our study,1720 students who screened positive for depression were included as research subjects.We calculated frequency and percentage for characteristics and anticipated stigma items.Independent sample t-test and one-way ANOVA were used to compare the anticipated stigma among different groups.Multivariable linear regression was used to assess the association of PHQ-9 with anticipated stigma.Results:There was anticipated stigma among medical students with depression.Among these students,the stigma of teachers and during the application of a residency or master′s degree was concerned most.The anticipated stigma of students who are male,Han ethnic group,major in psychiatry,have clinical practice experience and family history of mental illness,and come from cities and towns is higher(P<0.05).PHQ-9 scores were positively correlated with stigma scores(P<0.001).Conclusion:Medical students with depressive symptoms worried about the stigma around them.Further research should be carried on to explore stigma in different groups(students,professors,and staff)and develop more targeted interventions.
