第三代双源螺旋CT冠状动脉成像评估糖尿病对冠状动脉斑块的影响

Impact of diabetes mellitus on coronary artery plaques assessed by the third generation dual source computed tomography coronary angiography

摘要 目的通过第三代双源螺旋CT冠状动脉成像评估糖尿病对冠状动脉斑块的影响。方法行第三代双源螺旋CT冠状动脉成像的患者213例分为糖尿病组72例和无糖尿病组141例,比较两组的冠状动脉钙化积分、冠状动脉重度狭窄率、易损斑块发生率、冠状动脉斑块类型和冠状动脉斑块量化特征(平均最大横截面积、平均斑块负荷、平均重构指数)。结果糖尿病组冠状动脉平均钙化积分(253.9±32.5)分,无糖尿病组(86.8±10.3)分,差异有统计学意义(t=3.233,P<0.05)。糖尿病组冠状动脉重度狭窄率38.9%,无糖尿病组21.3%,差异有统计学意义(χ^(2)=7.461,P<0.05)。糖尿病组易损斑块发生率45.8%,无糖尿病组22.7%,差异有统计学意义(χ^(2)=12.034,P<0.05)。糖尿病组钙化斑块发生率41.1%,无糖尿病组28.2%,差异有统计学意义(χ^(2)=6.920,P<0.05)。糖尿病组混合型斑块发生率36.7%,无糖尿病组30.9%,差异无统计学意义(χ^(2)=1.392,P>0.05)。糖尿病组非钙化斑块发生率22.2%,无糖尿病组40.9%,差异有统计学意义(χ^(2)=14.618,P<0.05)。糖尿病组斑块最大横截面积、斑块负荷、重构指数分别为(6.35±2.51)mm2、(58.4±13.1)%、(1.23±0.28),均显著高于无糖尿病组(4.60±2.03)mm2、(33.8±11.7)%、(1.05±0.19),差异均有统计学意义(t=2.125、2.598、2.012,P<0.05)。结论第三代双源螺旋CT能够准确评价糖尿病对冠状动脉粥样硬化斑块的影响,糖尿病患者较无糖尿病患者冠状动脉钙化积分、重度狭窄率、易损斑块发生率高,斑块量化指标(平均最大横截面积、平均斑块负荷、平均重构指数)高,钙化斑块比例更高。 Objective To evaluate the impact of diabetes mellitus on coronary artery plaques using the third generation dual source computed tomography coronary angiography(CCTA).Methods A total of 213 patients including 72 with diabetes mellitus and 141 controls without diabetes mellitus who underwent dual source CCTA were studied.Coronary artery calcification score,severe stenosis rate,vulnerable plaque rate,plaque classification,plaque area,plaque burden and remodeling index was compared between the two groups.Results The average coronary artery calcification score was(253.9±32.5)and(86.8±10.3)for diabetes mellitus group and control group,with significant difference between groups(t=3.233,P<0.05).The severe stenosis rate was 38.9%and 21.3%for diabetes mellitus group and control group,with significant difference between groups(χ^(2)=7.461,P<0.05).The vulnerable plaque rate was 45.8%and 22.7%for diabetes mellitus group and control group,with significant difference between groups(χ^(2)=12.034,P<0.05).The calcified plaque rate was 41.1%and 28.2%for diabetes mellitus group and control group,with significant difference between groups(χ^(2)=6.920,P<0.05).The mixed plaque rate was 36.7%and 30.9%for diabetes mellitus group and control group,without significant difference between groups(χ^(2)=1.392,P>0.05).The non-calcified plaque rate was 22.2%and 40.9%for diabetes mellitus group and control group,with significant difference between groups(χ^(2)=14.618,P<0.05).The plaque area,plaque burden and remodeling index was(6.35±2.51),(58.4±13.1),(1.23±0.28)for diabetes mellitus group,while that for control group was(4.60±2.03)、(33.8±11.7)、(1.05±0.19),with significant difference between groups(t=2.125,2.598,2.012,P<0.05).Conclusions The third generation dual-energy CT coronary angiography can accurately evaluate the impact of diabetes mellitus on coronary artery plaques.Compared with patients without diabetes mellitus,diabetes mellitus patients have higher calcification score,severe stenosis rate,vulnerable plaque rate,calcified plaque rate,plaque area,plaque burden and remodeling index.
