摘要
目的研究血小板平均容积结合病理特征预测急性心肌梗死患者预后的价值。方法回顾性分析2017年2月—2019年3月于首都医科大学大兴教学医院心血管内科收治的186例急性心肌梗死患者的临床资料,观察患者在住院期间及出院后6个月主要不良心血管事件(MACE)发生情况,并采用多因素Logistic回归分析法分析患者发生MACE的独立影响因素。结果186例患者在院内及出院后随访6个月期间,发生MACE 37例,占19.89%;未发生MACE 149例,占80.11%。多因素Logistic回归分析显示,年龄(OR=1.239,95%CI=1.026,1.496)、Gensini积分(OR=1.767,95%CI=1.433,2.179)、甘油三酯(OR=1.351,95%CI=1.057,1.727)、血小板平均容积(OR=1.425,95%CI=1.107,1.834)是急性心肌梗死患者发生MACE的独立影响因素(P<0.05)。结论血小板平均容积及年龄、Gensini积分、甘油三酯是急性心肌梗死患者发生MACE的独立影响因素,临床应重视早期干预。
Objective To study the value of mean platelet volume combined with pathological features to predict the prognosis of patients with acute myocardial infarction.Methods The clinical data of 186 patients with acute myocardial infarction admitted to the Department of Cardiovascular Medicine of Daxing Teaching Hospital of Capital Medical University from February 2017 to March 2019 were retrospectively analyzed,and the main adverse cardiac symptoms during the hospital stay and 6 months after discharge were observed.The occurrence of vascular events(MACE),and the independent factors of MACE were analyzed by multivariate logistic regression analysis.Results In 186 patients,MACE occurred in 37 cases during the follow-up period and 6 months after discharge,accounting for 19.89%;149 cases did not occur,accounting for 80.11%.Multivariate Logistic regression analysis showed that age(OR=1.239,95%CI=1.026~1.496),Gensini score(OR=1.767,95%CI=1.433~2.179),triglycerides(OR=1.351,95%CI=1.057~1.727),and mean platelet volume(OR=1.425,95%CI=1.107~1.834)are the independent influencing factors of MACE in patients with acute myocardial infarction(P<0.05).Conclusion The average platelet volume and age,Gensini score,and triglycerides are independent influencing factors of MACE in patients with acute myocardial infarction.Early intervention should be valued in clinical practice.
